Introduction. La loi Hôpital Patient Santé Territoire (21/07/09) introduit l'inter-professionnalité. Nous voulions recueillir l'avis des médecins généralistes sur le transfert de certaines de leurs compétences aux infirmiers, pharmaciens, et sages-femmes. Méthode. Enquête transversale descriptive déclarative multicentrique par questionnaire anonyme numérique auprès de 2416 médecins généralistes libéraux des régions Auvergne-Rhône-Alpes et Limousin, du 07/06/17 au 07/08/17. Résultats. Parmi les 281 questionnaires exploités, les médecins généralistes avaient une durée d'exercice de 16.6 ans en moyenne, en milieu rural (33.1%), semi-rural (31.3%), et urbain (35.6%). Parmi eux, 67.6% effectuaient régulièrement des consultations de gynécologie/contraception. Ils étaient favorables aux tests réalisables en pharmacie à 64.1% pour le diabète, 45.2% pour la grippe, et 31.3% pour l'angine streptococcique. Ils étaient plutôt défavorables à la vaccination antigrippale en pharmacie (moyenne sur l'échelle de Likert 1-10 : 4.7), et au rôle de pharmacien correspondant (3.3). Le résultat était mitigé pour l'entretien pharmaceutique (5.4), et les substituts nicotiniques prescrits par les infirmiers (5.4). Ils étaient plutôt favorables à l'apparition d'infirmiers de pratique avancée (6.1), et au suivi gynécologique/contraception des sages-femmes (6.6). Conclusion. L'inter-professionnalité pourrait revaloriser la médecine générale, notamment dans les maisons de santé pluridisciplinaires. Le risque serait la complexification des consultations. L'opinion des médecins généralistes pouvant évoluer, l'enquête devrait être répétée.

Contexte : en 2014, avec la création de la sous-section 53-03 Médecine Générale au Conseil National des Universités, cette discipline confirme sa reconnaissance universitaire. Si chaque médecin généraliste apporte des données utilisables pour la recherche, seule une minorité d'entre eux, y participe activement. Objectif principal : explorer les opinions des médecins généralistes qui participent à la recherche. Méthode : étude qualitative avec analyse thématique par entretiens semi-dirigés. Recrutement raisonné en variation maximale de médecins généralistes participant ou ayant participé à la recherche. Vingt-deux entretiens semi-dirigés ont été réalisés auprès de médecins de dix départements de France, entre juin 2016 et avril 2017, jusqu'à saturation des données. Résultats : les médecins généralistes interrogés étaient impliqués à des degrés divers dans la recherche. Ils étaient motivés par la volonté de développer leur spécialité. Pour cela, ils avaient un rôle de transmission auprès des étudiants, et de leurs confrères, rappelant l'importance des publications. Ils avaient le souci d'être reconnus dans leur travail. Cette reconnaissance impliquait la formation à la recherche et l'autonomie financière. Pour être efficient, les médecins expliquaient vouloir une recherche en accord avec leurs convictions et celles de leurs patients. Pour poursuivre le développement de cette recherche leur semblaient nécessaire : le travail d'équipe, avec des réseaux pérennes, une recherche adaptée à la pratique spécifique du généraliste et l'utilisation du DPC, pour son intérêt budgétaire et l'assurance d'une autonomie financière. Conclusion : les opinions des médecins généralistes participant à la recherche varient en fonction de leur degré d'implication. Allier les trois piliers de la discipline universitaire : le soin, l'enseignement et la recherche, semble possible au travers des entretiens réalisés.

Dialogue interpreting includes what is variously referred to in English as Community, Public Service, Liaison, Ad Hoc or Bilateral Interpreting - the defining characteristic being interpreter-mediated communication in spontaneous face-to-face interaction. Included under this heading are all kinds of professional encounters: police, immigration and welfare services interviews, doctor-patient interviews, business negotiations, political interviews, lawyer-client and courtroom interpreting and so on. Whereas research into conference interpreting is now well established, the investigation of dialogue interpreting as a professional activity is still in its infancy, despite some highly promising publications in recent years. This special issue of The Translator, guest-edited by one of the leading scholars in translation studies, provides a forum for bringing together separate strands within this developing field and should create an impetus for further research. Dialogue interpreting is a generic term covering a diverse range of fields of interpreting which have in common the basic feature of face-to-face interaction between three parties: the interpreter and (at least) two other speakers. The interaction consists of spontaneous dialogue, involving relatively short turns at talk, in two languages. It is usually goal-directed in the sense that there is some outcome to be negotiated. The studies in this volume cover several different fields: courtroom interpreting, doctor-patient interviews, immigration interviews, etc., and involve a range of different languages: Spanish, Portuguese, Polish, More and Austrian Sign Language. They have in common that they view the interpreter as just one of the parties to this three-way exchange, in which each participant's moves can affect each other participant and thus the outcome of the event. In Part I, new research directions are explored in studies which piece together evidence of the ways dialogue interpreters actually behave and the effects of their behaviour. This is followed by two studies which discuss traditional interpreter roles - the 'King's Linguist' in Burkina Faso and the Oranda Tsûji, official interpreters employed in isolationist eighteenth-century Japan to ensure contact with the outside world. Finally, issues involved in training are the subject of two chapters relating to Austria and the UK. The variety of aspects and approaches represented in the volume - linguistic, cultural, pragmatic, historical - offer a rich and fascinating overview of the field of dialogue interpreting studies as it now stands.
