Nephrotoxicity : Otoxicity of Drugs. Ce livre rassemble les textes des communications présentées au symposium de Rouen (mai 1981) ayant pour thème l'étude de la néphrotoxicité des antibiotiques. La majeure partie de l'ouvrage est réservée à la néphrotoxicité des aminoglycosides. Sont détaillés les aspects morphologiques, les résultats d'études autoradiographiques, d'études de fractionnement cellulaires permettant l'analyse du comportement des mitochondries ou des lysosomes. Une large part est réservée aux variations de multiples enzymes urinaires excrétées sous l'action des médicaments. Le rôle de nombreux facteurs est largement discuté : influence des doses injectées, du rythme d'administration, de la durée du traitement, de l'âge, de l'existence d'une néphropathie antérieure. À la lecture des différentes études présentées, le clinicien pourra juger des différences de potentiel néphrotique des antibiotiques qui lui sont proposés pour le traitement des infections sévères. La place consacrée à l'otoxicité des aminoglycosides est plus restreinte mais les études de variations de potentiels pour détecter les signes d'otoxicité, les études de pharmacocinétique des aminogly¬cosides sont des contributions fort intéressantes et très originales.

The majority of the offending toxicants to be reviewed in this volume were devel oped to help mankind, and it is only with prolonged or widespread application that their adverse effects have been recognized. Conversely, in the case of pre scrip tion drugs, there has been an attempt to identify the adverse effects in advance and incorporate these risks into the decision of approval for human consumption. Unfortunately, for those drugs in which recognized injury occurs only after prolonged use, such appraisals are made in retrospect. Despite this, most renal injury induced by drugs or toxicants can be either prevented by excluding drugs with unacceptable side effects or interrupted by eliminating the offending agent once damage is manifested. The fact that prevention, reversibility, or arrest of renal injury is possible provided a major impetus for this publication. Since no international registry for nephrotoxic injury exists, estimates of incidence must rely on less than ideal sources. Recently I, together with Dr. William Bennett, summarized a survey of the frequency of various categories of nephrologic disease (Porter and Bennett, 1981). Based on this survey, we projected that in nearly one of ten patients seeking nephrologic consultation a nephrotoxic etiology may be involved. Of cases of end-stage renal disease, between 3 and 4% are due to drug nephrotoxicity, according to recent published results(European Dialysis and Transplant Association, 1979). For acute renal failure, antibiotics and contrast agents persist as major offending agents, while for chronic renal failure, analgesics remain a worldwide problem.

Over the years a number of excellent books have classified and detailed drug drug interactions into their respective categories, e.g. interactions at plasma protein binding sites; those altering intestinal absorption or bioavailability; those involving hepatic metabolising enzymes; those involving competition or antagonism for receptor sites, and drug interactions modifying excretory mechanisms. Such books have presented extensive tables of interactions and their management. Although of considerable value to clinicians, such publica tions have not, however, been so expressive about the individual mechanisms that underlie these interactions. It is within this sphere of "mechanisms" that this present volume specialises. It deals with mechanisms of in vitro and in vivo, drug-drug, drug food and drug-herbals interactions and those that cause drugs to interfere with diagnostic laboratory tests. We believe that an explanation of the mechanisms of such interactions will enable practitioners to understand more fully the nature of the interactions and thus enable them to manage better their clinical outcome. If mechanisms of interactions are better understood, then it may be pos sible for the researcher to develop meaningful animal/biochemical/tissue cul ture or physicochemical models to which new molecules could be exposed during their development stages. The present position, which largely relies on patients experiencing adverse interactions before they can be established or documented, can hardly be regarded as satisfactory.
