Les Permanences d'Accès aux Soins de Santé (PASS) ont pour mission de repérer les patients démunis pour leur proposer une prise en charge globale à l'hôpital. Suite au constat du recours fréquent aux urgences des patients précaires, l'équipe de la PASS des urgences d'Angers utilise de façon empirique le critère « multiple recours à l'urgence » comme aide au repérage. Objectif : évaluer la pertinence de ce critère en réalisant une étude rétrospective des dossiers des patients ayant eu un multiple recours au Service d'Accueil des Urgences (SAU) d'Angers entre le 1er janvier et le 30 juin 2006. Analyser les caractéristiques médicosociales de la population précaire fréquentant le SAU. Matériel et méthode : les critères d'inclusion étaient au moins deux passages en un mois ou trois passages en trois mois aux urgences. Les dossiers des patients précaires ou possiblement précaires ont été revus et analysés avec l'assistante sociale de la PASS. Résultats : 1069 patients ont eu un multiple recours aux urgences (5% des admissions), 297 étaient en situation précaire (28 %). Les patients précaires étaient plus jeunes, plus souvent des hommes et avaient plus souvent des antécédents d'addiction à l'alcool et psychiatriques que les patients non précaires. Pendant la période, 132 patients précaires avaient été pris en charge par la PASS dont 1/3 avec multiples recours (Odds Ratio 9,4). Conclusion : le critère "multiple recours" semble pertinent pour détecter les situations de précarité aux urgences.Son association aux caractéristiques médicosociales de la population précaire et son intégration au système informatisé des urgences pourrait améliorer l'efficacité du repérage par les soignants.
