Introduction : en 2018, un médecin sur dix poursuit une activité professionnelle après l'âge de 65 ans. La retraite est une étape importante, et souvent attendue dans une vie professionnelle. Nous nous sommes donc interrogés quant aux motivations et aux ressentis de ces médecins généralistes poursuivant leur activité professionnelle au-delà de l'âge légal de départ à la retraite. Méthode : d'octobre 2017 à janvier 2018 nous avons rencontré 11 médecins généralistes Varois âgés de plus de 65 ans, et toujours en activité, dans le cadre d'une étude qualitative. Une grille d'entretien préalablement établie a permis de réaliser des entretiens semi-dirigés. Une analyse par codage ouvert des verbatim a été entreprise dans les suites. Résultats : les motivations à la poursuite d'une activité professionnelle évoqués durant cette étude sont multiples. Un aspect financier en lien avec des évènements et/ou des choix de vie sont avancés. Le manque de successeur est cependant un thème récurrent qui inquiète, voire révolte, le médecin généraliste. Une relation médecin-malade forte, une responsabilité déontologique et morale poussent certains confrères à ne pas s'arrêter. Conclusion : cette étude a permis d'identifier le manque de successeur comme un des freins à une cessation d'activité. Une réflexion et un travail sur cette transmission de savoir ainsi que de la patientèle devrait être mené. Un lien plus fort avec les jeunes générations semble être une solution clé à cette problématique.

Introduction : Il semble que le report de quelques années de l'âge de départ en retraite des médecins pourrait être l'une des meilleures solutions à court terme pour atténuer la pénurie actuelle de médecins généralistes. Qu'en pensent ces médecins qui ont atteint l'âge de la retraite ? Quelles sont les motivations et les freins à poursuivre une activité après 65 ans ? Quelles mesures pourraient les encourager à rester en activité, en particulier en soins de premier recours ? Méthode : Une enquête qualitative sous forme d'entretiens semi-dirigés a donc été menée en 2011 auprès de 13 médecins généralistes libéraux picards volontaires. Agés de 65 à 68 ans, ils représentent un panel de différents parcours après 65ans, entre retraite complète et poursuite d'activité, libérale, salariée ou de remplacements.Résultats : La quasi-totalité des médecins rencontrés est favorable à la poursuite d'une activité après 65ans comme une transition avant la retraite. Une majorité recherche une activité réduite et peu contraignante qui reste rentable financièrement, ce qui est impossible en secteur libéral (au cabinet ou en remplacements) du fait de charges financières inadaptées, mais possible en secteur salarié. Les poursuites tardives d'activité libérale dépendent de l'état de santé, du milieu et du mode d'exercice, de la façon dont la retraite est appréhendée, du regard porté sur le métier en fin de carrière, et des possibilités de succession.Discussion : Nos résultats sont en accord avec des études nationales et internationales, et notamment Canadiennes, où la même pénurie médicale existe. Apporter une rémunération spécifique aux médecins en fin de carrière, leur assurer des conditions d'exercice souples et non contraignantes en secteur libéral, accompagner les médecins isolés dans des démarches de regroupement et assurer la survie des cabinets de groupe déjà existants semblent primordiaux pour leur faciliter un exercice réduit et augmenter leur chance de succession.Conclusion : Les mesures qui faciliteraient la poursuite d'activité des médecins retraités recoupent étroitement celles qui encourageraient les installations de jeunes médecins. Quoi de plus rassurant et formateur pour un jeune médecin que de commencer sa carrière aux côtés de médecins aussi passionnés qu'expérimentés ?

Health and social care in every system and in every country is labour intensive, and must be oriented to people's needs if it is to be effective. It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. In the late 1950s, like tens of thousands of young men of his generation, Pierre Bourdieu, having recently passed the agrégation in philosophy, found himself immersed in the Algerian war. Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. Jean-Baptiste Say (1767–1832) was one of the first great economists to have laid down the foundations of economic science. Author of the famous Treatise on Political Economy in 1803, which was revised and re-edited on several occasions, he published numerous other works including a voluminous Complete Course in Practical Political Economy in 1828–9. He also taught political economy successively from 1815 until his death in three Parisian establishments: the Athénée, the Conservatory of Arts and Trades, and the Collège de France. The texts in which Say exposes his approach to political economy have not been available in the English language until now except for the fourth edition of the ‘Preliminary Discourse’ which serves as an introduction to the Treatise. This book presents a translation which renders his works accessible to the English speaking world. For the first time, English readers will be able to become directly immersed in Say’s principal texts, where he develops his conception of political economy. Jean-Baptiste Say and Political Economy proposes a translation of a selection of eleven of Say’s texts. The first three are versions of the ‘Preliminary Discourse’ from the Treatise’s editions of 1803, 1814 and 1826 with the variations of the editions of 1817, 1819 and 1841. The following four texts are the opening discourses pronounced at the Conservatory in 1820 and 1828 and the Collège de France in 1831 and 1832. The eighth text is the ‘General Considerations’ which open the Complete Course in Practical Political Economy of 1828, with the variations of the 1840 re-edition. The final three texts are those Say devotes to ‘the progress of political economy’ in what is akin to a history of economic thought. This volume is of great importance to economic historians and people studying Jean-Baptiste Say, as well as those who are interested in economic theory and philosophy and political economy.

Much orthodox economic theory is based on assumptions which are treated as self-evident: supply and demand are regarded as independent entities, the individual is assumed to be a rational agent who knows his interests and how to make decisions corresponding to them, and so on. But one has only to examine an economic transaction closely, as Pierre Bourdieu does here for the buying and selling of houses, to see that these abstract assumptions cannot explain what happens in reality. As Bourdieu shows, the market is constructed by the state, which can decide, for example, whether to promote private housing or collective provision. And the individuals involved in the transaction are immersed in symbolic constructions which constitute, in a strong sense, the value of houses, neighbourhoods and towns. The abstract and illusory nature of the assumptions of orthodox economic theory has been criticised by some economists, but Bourdieu argues that we must go further. Supply, demand, the market and even the buyer and seller are products of a process of social construction, and so-called ‘economic' processes can be adequately described only by calling on sociological methods. Instead of seeing the two disciplines in antagonistic terms, it is time to recognize that sociology and economics are in fact part of a single discipline, the object of which is the analysis of social facts, of which economic transactions are in the end merely one aspect. This brilliant study by the most original sociologist of post-war France will be essential reading for students and scholars of sociology, economics, anthropology and related disciplines.
