In 1998, approximately 30 million people worldwide were living with HIV/AIDS, about 5 million of whom became infected that year. The epidemic continues to expand, with an estimated doubling time of 10 years, making AIDS the leading infectious cause of death ahead of tuberculosis and malaria. Even in the U.S.A. where the death rate from AIDS is declining as a result of effective drug therapies, HIV infection rates continue to climb in several population groups. The prevalence of AIDS among people over the age of 50 is steadily increasing, and most older people are unprepared to address it for a number of reasons, including the widespread discomfort with matters sexual and homosexual and the belief that elderly people are not sexually active and therefore not at risk. The number of women living with HIV/AIDS is increasing steadily. Very few research studies have been done to understand the experiences of older women living with the disease. The purpose of my study was to understand the challenges faced by midlife women living and aging with HIV/AIDS through the progression of their illness. Exploratory case studies were developed of three women between the ages of 50-55 years of age. These women are only in their early midlife, but because of their risky life style factors, HIV treatment side effects and other comorbidities are dealing with disabilities, emotional impacts, difficult lifestyle adjustments, and decline in functional capacities. They are unable to use services provided for older adults and are excluded because of construction of age categories in providing services. We need to adopt a life course approach in providing community and healthcare services for these women, as their roles and challenges change in living and aging with HIV. The elements of the Chronic Care Model were applied to suggest how the quality of life of these women could be enhanced as their needs change throughout their lives.

With the development of effective antiretroviral therapies (ART) in the mid-1990s, HIV became a treatable although serious condition, and people who are adherent to HIV medications can attain normal or near-normal life expectancies. Because of the success of ART, people 50 and older now make up a majority of people with HIV in high-income countries and other places where ART is accessible. The aging of the HIV epidemic is a global trend that is also being observed in low- and middle-income countries, including countries in sub-Saharan Africa, where the greatest number of older people with HIV reside (3.7 million). While globally over half of older adults with HIV are in sub-Saharan Africa, we have little information about the circumstances, needs, and resiliencies of this population, which limits our ability to craft effective policy and programmatic responses to aging with HIV in this region. At present, our understanding of HIV and aging is dominated by information from the U.S. and Western Europe, where the epidemiology of HIV and the infrastructure to provide social care are markedly different than in sub-Saharan Africa. Aging with HIV in Sub-Saharan Africa addresses this gap in our knowledge by providing current research and perspectives on a range of health and psychosocial topics concerning these older adults from across this region. This volume provides a unique and timely overview of growing older with HIV in a sub-Saharan African context, covering such topics as epidemiology, health and functioning, and social support, as well as policy and program implications to support those growing older with HIV. There are very few published volumes that address HIV and aging, and this is the first book to consider HIV and aging in sub-Saharan Africa. Most publications in this area focus on HIV and aging in Uganda and South Africa. This volume broadens the scope with contributions from authors working in West Africa, Botswana, and Kenya. The range of topics covered here will be useful to professionals in a range of disciplines including psychology, epidemiology, gerontology, sociology, health care, public health, and social work.

Black women, ages 45 and older, are disproportionately affected by the HIV/AIDS epidemic. In 2001, Black women comprised 11% of all older women in the United States; however, they accounted for more than 50% of AIDS cases and more than 65% of HIV cases (Winningham et al., 2004). The literature review reflected little research specifically looking at the reason(s) for the increase in HIV in Black women, ages 45 to 60. Furthermore, few research studies focused on HIV patient health beliefs and their experiences in living with HIV. This study was conducted with an ethnography research method coupled with the American Black feminist theory. The primary purpose of this qualitative study was to examine the reason(s) why the HIV rate is growing at an alarming pace for Black women ages 45 to 60 years old. The research instruments consisted of a demographic form and a six-question interview guide that formed the basis of 60–90 minute interviews meeting the research criteria. Demographic information was collected from a self-administered survey that included age, employment status, income, education level, date of infection, source of infection, date treatment started, whether the participant was currently in treatment or not in treatment, and the reasons for that treatment decision. This was a non-random sample of convenience. This researcher chose not to conduct a random sample because of the parameters of the inclusion and exclusion criteria. The participants consisted of 22 HIV-positive midlife Black women between the ages of 45 and 60 years old who self-selected to participate in the research study; however, two of the participants declined to have their interviews included in the results. Therefore, 20 participant-encounters were included. IRB approval was obtained from the University of San Diego. The participants signed an informed consent that insured their confidentially. The researcher analyzed the qualitative information by hand coding the data because the ethnic, feminist, and culture elements might lead to misinterpretation if qualitative-analysis software was used. Black women, ages 45 to 60 years offered significant reasons why they were infected with the HIV virus (e.g., lack of education about HIV, media influences, aging, unprotected sex, the onset of drug use after age 40). The findings from this study remain pertinent in all nursing research and advance nursing's utilizing of the American Black feminist theory. Finally, the findings may also facilitate further research into the reason(s) for the increase in HIV infections in Black midlife women. In addition, this study may further the body of research which leads to changes in the delivery and policies of health care.
