Book Description
Cardiovascular, respiratory, and related conditions cause more than 40 percent of all deaths globally, and their substantial burden is rising, particularly in low- and middle-income countries (LMICs). Their burden extends well beyond health effects to include significant economic and societal consequences. Most of these conditions are related, share risk factors, and have common control measures at the clinical, population, and policy levels. Lives can be extended and improved when these diseases are prevented, detected, and managed. This volume summarizes current knowledge and presents evidence-based interventions that are effective, cost-effective, and scalable in LMICs.

Book Description
Advanced ischemic heart disease is fast becoming one of the most challenging problems facing the modern cardiovascular physician and current established therapies often fail to adequately address this population of patients. As therapy of heart disease evolves, we need to address the challenging questions posed by this clinical problem. Treatment of Advanced Ischemic Heart Disease brings together the most recent advances in surgical techniques, protection of the heart and postoperative care, to aid the study and treatment of these patients who deserve the best that modern medicine can provide them. The diagnosis and treatment of coronary artery disease and heart failure has now been standardized to the point that there are very well established guidelines promoted by the American Heart Association and The American College of Cardiology. This text will pick up where the guidelines leave off and address the real challenge of patients with advanced ischemic heart disease. Though therapy is clearly challenging, new advances are making effective therapy of these patients truly possible. This text embodies that notion. Finally, there is much still to learn about the basic mechanisms of the progression of heart failure due to coronary artery disease and how best to assess these therapeutic options. The text will address these with a focus on treatment so that it may be a practical reference for the practicing clinician as well as the basic or clinical researcher and student.

Book Description
This book provides an overview of the etiology of coronary artery disease and focuses on the main therapies and drug interventions currently available. It highlights drug pharmacology and therapeutic challenges, with a special emphasis on the underlying principles of available therapeutics and the on-going development of drugs for coronary artery disease. The book is divided into eight chapters, the first of which describes the classical mechanism of coronary artery disease and its clinical presentations. Chapter 2 lists the principles of and related evidence on the prevention and treatment of coronary artery disease. This includes diet and lifestyle management, and guidelines for the treatment of acute coronary syndrome and stable coronary artery disease. In turn, Chapter 3 describes revascularization treatments, covering percutaneous coronary interventions, coronary artery bypass grafts and thrombolysis. This chapter also addresses the main therapeutic challenges. The following chapters provide an overview of three major categories of coronary artery disease drugs, which target thrombosis (Chapter 4), lipid metabolism (Chapter 5), and hypertension (Chapter 6). Heart failure constitutes the major health burden in the late stage of coronary artery disease; accordingly, current heart failure therapeutics and related drug pharmacology are described in Chapter 7. In closing, Chapter 8 provides a summary of on-going clinical trials for coronary artery disease drug development and discusses a number of promising areas in which intensive research into new therapeutics is being pursued. Given its scope, the book will be of interest to clinicians, medical students and research scientists, as well as to pharmaceutical professionals who are seeking new therapies and drugs for coronary artery disease and related disorders.

Book Description
Despite remarkable advances in the understanding and management of impaired left ventricular function (LVF) and related coronary disease, the prevalence of heart failure in the U.S. and the resulting death rates have almost tripled in the past 3 decades. New understanding of the relationships between the myocardium and LVF demonstrate a direct correlation between myocardial viability and improved patient survival. Because of this, myocardial viability is now a major investigative area in contemporary cardiology, one that holds significant clinical and prognostic relevance. Authored by physicians of international renown, the book brings together various disciplines affecting myocardial viability, with five main sections providing an introduction and comprehensive review of: basic concepts and mechanisms; vascular biology and cellular physiology; advances in functional imaging; and perfusion, metabolism and cell membrane integrity. New concepts, such as stunning and hibernation, are clarified, and subsequent novel diagnostic and therapeutic strategies are described. New and sophisticated examination techniques are also presented, as well as advances in instrumentation and imaging techniques, which may result in improved use of resources and enhanced efficiency of health care delivery. This monograph will serve as a reference source for those interested in the field of myocardial viability, and hopefully improve understanding between investigators from various disciplines. Clinical cardiologists, physicians, and nurses in the field, as well as radiologists, vascular surgeons, reperfusionists, cellular biologists and physiologists, and students will all find material of interest in this book.

Book Description
This book is intended for general cardiologists and other physicians involved in the care of patients with chronic stable angina (CSA). The goal of this book is to update clinicians on recent data on the medical management of patients with CSA. Ischemic heart disease remains a major public health problem. Chronic stable angina is the initial manifestation of ischemic heart disease in approximately one half of patients. Stable coronary artery disease is generally characterized by episodes of reversible myocardial demand/supply mismatch, related to ischaemia or hypoxia, which are usually inducible by exercise, emotion or other stress and reproducible—but, which may also be occurring spontaneously. Such episodes of ischaemia/hypoxia are commonly associated with transient chest discomfort (angina pectoris). The aim of the management of CSA is to reduce symptoms and improve prognosis. The management of these patients encompasses lifestyle modification, control of coronary artery disease risk factors, evidence-based pharmacological therapy and patient education. All patients with stable angina should be offered optimal medical treatment, defined as one or two anti-anginal drugs as necessary, plus drugs for secondary prevention of cardiovascular disease. Regarding the role of revascularization, randomised trials provide compelling evidence that myocardial revascularisation by coronary artery bypass grafting or by percutaneous coronary intervention improves symptoms of angina relative to continued medical treatment.

Book Description
In the past two decades a number of studies have shown that abnormalities in the function and structure of coronary microcirculation can be detected in several cardiovascular diseases. On the basis of the clinical setting in which it occurs, coronary microvascular dysfunction (CMD) can be classified into four types: CMD in the absence of any other cardiac disease; CMD in myocardial diseases; CMD in obstructive epicardial coronary artery disease; and iatrogenic CMD. In some instances CMD represents an epiphenomenon, whereas in others it represents an important marker of risk or may contribute to the pathogenesis of myocardial ischemia, thus becoming a possible therapeutic target. This book provides an update on coronary physiology and a systematic assessment of microvascular abnormalities in cardiovascular diseases, in the hope that it will assist clinicians in prevention, detection and management of CMD in their everyday activity.

Book Description
For many years, there has been a great deal of work done on chronic congestive heart failure while acute heart failure has been considered a difficult to handle and hopeless syndrome. However, in recent years acute heart failure has become a growing area of study and this is the first book to cover extensively the diagnosis and management of this complex condition. The book reflects the considerable amounts of new data reported and many new concepts which have been proposed in the last 3-4 years looking at the epidemiology, diagnostic and treatment of acute heart failure.
