This volume includes contributions of renowned experts and is based on a selection of papers presented at the Second International Conference on Malignant Lymphomas that was held in Lugano, Switzerland, in June 1984. Updated information is provided on various experimental fields including cell biolo gy, immunology, genetics and cell biochemistry. Particular attention has been given to special clinical entities such as the lymphomas associated with the Acquired Immune Deficiency Syndrome. Recent advances in the treat of Hodgkin's disease, ncin-Hodgkin's lymphomas and childhood lym ment phomas are highlighted with emphasis on growing experience with new therapeutic approaches including monoclonal antibodies and bone marrow transplantation. The contents of the volume reflect and translate numerous avenues of exploration. While there might be some unavoidable overlap, most appear essentially complementary. The intent of the Program Committee of the Second International Conference on Malignant Lymphomas was to promote further interactions between all those who are devoting major efforts to elucidate the nature of these diseases or their optimal therapy. This volume will hopefully amplify this objective. The Editors wish to thank the contributors for delivering the manuscripts on schedule.

Fernando Cabanillas In 1993, Fisher et al. published the results of a randomized trial comparing three third-generation regimens against the classic CHOP combination. For several years, the oncology community had been convinced that the third generation regimens were clearly superior to CHOP. It came as a shock to many that there was no difference in outcome between the four arms of this clinical trial. The logical conclusion is that CHOP is as good as any of the other regimens tested in that study. Unfortunately, this excellent study has been misinterpreted by many as proving that there has not been any progress in the field of lymphoma during the last 20 years. Furthermore, it has led to a fatalistic attitude in the reasoning of many clinicians who feel that 'nothing works better than CHOP' and therefore that it is not worth testing new drugs or developing novel regimens. However, the process by which we move forward in the oncology field is seldom by dramatic breakthroughs. Frequently, what appears at first glance to be a breakthrough turns out later to be just a modest step forward. Several steps forward eventually add up to a major advance, but this advance goes unnoticed because of the slow nature of the process. In this volume, we have chosen to discuss several of these steps, which we feel are clearly making a positive impact on the field of lymphomas and which soon should make a major difference in therapeutic results.

This volume summarizes the contribu tions presented at the First International Symposium on Hodgkin's Disease that took place in Cologne (FRG) on October 2-3, 1988. There is little doubt that the Hodgkin and Reed-Sternberg (H and RS) cells and their variants represent the malignant population in Hodgkin's lymphoma; however, there is still a fierce debate as to the possible cell of origin of Hand RS cells. Many of the problems confounding earlier research into this question were related to the difficulty or virtual impossibility, of obtaining purified populations of Hand RS cells. Most of the recent progress stems from the establishment of permanent cell lines of Hand RS cells in culture.

This volume is intended to present an eclectic selection of informative reviews on aspects of the malignant lymphomas, including Hodgkin's disease. The choice of suitable review topics in this field is made hazardous by several facts. First, the growth of insight into basic mechanisms of lymphomagenesis is explosive, as is the expansion of our understanding of the need for precise morphologic classification of these diseases. Second, new therapeutic agents, cytotoxic and biologic, as well as new drug delivery strategies, are appearing with remarkable frequency. Finally, old "standard" approaches, such as the staging laparotomy for Hodgkin's disease, are being reconsidered and deserve attention in an up-to-date collection of papers. The 10 reviews presented here fall generally into three categories. First, the optimal diagnostic and therapeutic approaches to Hodgkin's disease are considered by Mark Leibenhaut, who reviews the status of the diagnostic staging laparotomy, and by Philip Bierman, who discusses his thoughts on the timing of high-dose treatment with bone marrow transplantation for this disease. There follow four reviews of current management of specific histologic subtypes of malignant lymphoma. First, Velasquez reviews prognostic factors in both intermediate and high grade lymphomas. Miller and Dahlberg review and update the Southwest Oncology Group experience in developing curative cytotoxic combination therapies for diffuse large-cell lymphomas.
