Introduction : Les troubles digestifs, et les douleurs abdominales, sont un motif fréquent de consultation aux urgences pédiatriques (POSU). La démarche diagnostique est guidée par l'âge de l'enfant, l'interrogatoire et l'examen clinique. L'échographie est actuellement un examen de choix en pédiatrie. L'objectif de cette étude est de décrire ses indications et sa place dans la prise en charge diagnostique des pathologies digestives, au POSU, hors contexte traumatique. Matériel et Méthodes : Ce travail est une étude rétrospective incluant des patients ayant bénéficié d'une échographie abdominale lors de leur passage au POSU du CHRU de Nancy, entre juillet et septembre 2014, et entre janvier et mars 2015. Nous avons relevé les diagnostics suspectés avant l'échographie, le diagnostic échographique et le diagnostic final retenu. Résultats : Nous avons inclus 221 patients (103 filles, 118 garçons), âgés de 3 mois à 18 ans. Le premier motif de consultation est la douleur abdominale aigüe. Le principal diagnostic suspecté est l'appendicite aigüe. 67% des diagnostics suspectés avant échographie sont des urgences chirurgicales. Dans 22% des cas, aucun diagnostic n'est clairement suspecté avant la réalisation de l'examen. Presque la moitié des échographies (47%) sont sans particularité. 23.8% des diagnostics suspectés sont confirmés. Conclusion : Devant une urgence abdominale, l'échographie est un élément clé du diagnostic final en confirmant ou infirmant une urgence chirurgicale. Les indications non étayées par la clinique n'aboutissent pas à un diagnostic échographique. Des moyens pour améliorer les indications de prescription de ces échographies sont possibles.
