Elle rapporte également les résultats obtenus par les " pionniers " de la chirurgie robot-assistée pédiatrique. Objectif : Cette étude monocentrique a pour objectif d'établir la courbe d'apprentissage des pyéloplasties et réimplantations urétérales réalisées sous coelioscopie robot-assistée dans un service de chirurgie pédiatrique. Pour cela, nous avons étudié l'évolution des durées opératoires pour 3 binômes de chirurgiens pour ces procédures sur la période correspondant à nos 10 premières années d'expérience. Méthodes : Il s'agit d'une étude rétrospective réalisée sur tous les patients pédiatriques opérés entre le 1er novembre 2011 et le 1er Mars 2022. Le critère d'inclusion était d'avoir eu une intervention pour syndrome de jonction ou reflux vésico-urétéral. Les enfants pour lesquels le suivi post-opératoire était insuffisant (inférieur à 3 mois) ont été exclu de l'étude. Le critère de jugement principal est l'évolution des durées opératoires sur une période de 10 ans. Les critères de jugements secondaires sont la durée d'hospitalisation, le taux de conversion chirurgicale, les complications post opératoire et les résultats à 1 an de la chirurgie. Résultats : 81 patients ont été opérés d'une pyéloplastie par coelioscopie robot assistée et 50 d'une réimplantation urétérale. L'âge médian était de 8,3 ans pour les pyéloplasties et 3 ans pour les réimplantations urétérales. La durée du temps opératoire moyen concernant la pyéloplastie était de 187,86 minutes et de 165,38 minutes pour la réimplantation urétéro-vésicale. Nous avons pu observer une amélioration significative du temps opératoire en fonction du temps ainsi que du temps d'hospitalisation pour les pyéloplasties mais pas pour les réimplantations urétérales. Conclusion : Malgré un accès restreint au robot chirurgical, cette étude a permis de montrer une véritable courbe d'apprentissage pour les chirurgiens opérant sous coelioscopie robot-assistée et ce au prix d'une sélection importante des patients pour ne conserver que deux indications principales chez l'enfant : la pyéloplastie et la réimplantation urétéro-vésicale. De plus, ce travail montre l'intérêt de former des chirurgiens experts qui eux-mêmes réussiront à former d'autres chirurgiens en début d'expérience.
