CONTEXTE : la loi du 13 août 2004 propose la coordination des soins comme pilier de la réforme relative à l'Assurance Maladie. Le médecin traitant se voit confier la mission de coordonner les soins et d'adresser celui-ci vers un spécialiste lorsque nécessaire. Le patient doit, sauf conditions précises, consulter en premier recours son médecin traitant, sous peine de pénalisation financière. Devant cette évolution des réformes et de la démographie, une évaluation non tarifaire mais basée sur les caractéristiques du médecin généraliste, du patient et du motif de consultation semblait nécessaire. Un second objectif a été la mise ne évidence de facteurs favorisants ou de freins à la coordination des soins et à la collaboration entre médecins généralistes et spécialistes. METHODE : Etude observationnelle et transversale : évolution des pratiques par un recueil de données de 250 consultations de cardiologie, endocrinologie et dermatologie à Aubagne (PACA-13), données recueillies auprès de patients vus pour la première fois par le spécialiste et adressées par leur généraliste en second recours, sur une période de cinq mois, de décembre 2012 à avril 2013. RESULTATS : Seulement 20% des consultations spécialisées concernent un premier avis demandé par le médecin traitant. L'étude des caractéristiques propres à ces patients et au médecin généraliste qui l'adresse ne retrouve pas de facteurs modifiant la prise ne charge et le parcours spécialisé enclenché. Le type de recours au spécialiste, le type de question posée, la présence ou non de bilan ou de traitement antérieurs ne modifient pas les pratiques ni des généralistes ni des socialistes de manière significative (p>0,05). Seuls les patients avec une pathologie chronique ou en Affection Longue Durée voient leur probabilité d'avoir un bilan ou un traitement avant le second recours augmentée. On note par ailleurs que les patients en Affection Longue Durée consultant pour la première fois le spécialiste entraient plus souvent dans un parcours de soins spécialisé à la suite de ce second recours (p=0,02). CONCLUSION : Les médecins généralistes trouvent donc difficilement leur place de pivot et de coordinateur, notamment des soins spécialisés. Pour améliorer le second recours par une consultation chez le médecin traitant, des pistes sont proposées : un échange d'informations médicales sût et exhaustif par messagerie sécurisée ou par l'efficience du Dossier Médical Personnalisé, des démarches de soins protocolisées, une consultation de premier recours valorisée comme une consultation à part entière et spécifique.
