La crise liée à la pandémie de Covid-19 rappelle la dépendance de la France et de l'Europe vis-à-vis de pays tiers pour l'approvisionnement pharmaceutique. De fait, les sites de recherche ou de production de médicaments se sont raréfiés en France, passant de 570 en 2009 à 520 en 2019. La majorité des fabricants de principes actifs certifiés conformes à la pharmacopée européenne (CEP) est désormais située en Asie : 25 % en Chine + 23 %en Inde + 7 % dans d'autres pays asiatiques, soit 55%en Asie contre 36 %en Europe, 5 % aux Etats-Unis, 4 %dans d'autres pays. Aujourd'hui, 80% des principes actifs composant les produits consommés en France proviennent donc d'Asie. Or, au cours des 10 dernières années, notamment sous l'effet contraire de cette concentration de la production et de l'augmentation de la demande mondiale, les ruptures de stocks ont été multipliées par 20 pour les traitements essentiels. Afin de pallier ces pénuries, la France et l'Europe se sont engagées à sécuriser leurs approvisionnements et ont lancé une relocalisation stratégique de la production pour retrouver une autonomie sanitaire. En France, des associations de patients ou autres parmi lesquelles ll UFC-Que Choisir et des représentants politiques ont avancé diverses solutions comme l'obligation de constituer d'importantes réserves -proposition ayant d'ailleurs abouti à l'instauration d'un stock de sécurité-, ou le développement d'une production publique, ou encore la création d'un pôle public du médicament. De leur côté, les industriels de santé ont proposé de nombreux projets basés sur le retour d'un «made in France» pour certains produits en tension ou essentiels.

What are public health services? Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. Drawing on anthropology, historical sociology and social-epidemiology, this multidisciplinary book investigates how pharmaceuticals are produced, distributed, prescribed, (and) consumed, and regulated in order to construct a comprehensive understanding of the issues that drive (medicine) pharmaceutical markets in the Global South today. Based on primary research conducted in Benin and Ghana, and additional data collected in Cambodia and the Ivory Coast, this volume uses artemisinin-based combination therapies (ACTs) against malaria as a central case study. It highlights the influence of the countries colonial and post-colonial history on their models for state regulation, production, and distribution, explores the determining role transnational actors as well as industries from the North but also and increasingly from the South play in influencing local pharmaceutical markets and looks at the behaviour of health care professionals and individuals. Stepping back, the authors then unpick the pharmaceuticalization process and the multiple regulations at stake by looking at the workings of, and linkages between, (biomedical health) pharmaceutical systems, (representatives of companies) industries, actors in private distribution, and consumer practices. Providing a thorough comparative analysis of the advantages and disadvantages of different pharmaceutical systems, it is an important contribution to the literature on pharmaceutalization and the governance of medication. It is of interest to students, researchers and policy-makers interested in medical anthropology, the sociology of health and illness, global health, healthcare management and pharmacy.
