Mais de nos jours, le principe de gratuité des services de santé est de plus en plus remis en cause. Divers modes de financement des services de santé ont été envisagés, à savoir : le paiement direct, le financement par les contributions publiques (l'impôt) et le recours aux aides extérieures (bilatérales et multilatérales). Les trois premiers modes de financement expérimentés jusque-là, ont montré leurs limites. Des réflexions s'orientent actuellement vers la conception et la mise en oeuvre d'un plan d'assurance maladie qui couvrirait l'ensemble ou, tout au moins, la majorité de la population. Cet état de choses appelle une série d'interrogations dont les principales sont les suivantes : Dans quelles conditions et selon quelles modalités l'assurance maladie peut-elle être mise en oeuvre en République du Congo ? L'organisation actuelle du système de santé peut-elle garantir des prestations nécessaires et répondre aux exigences requises dans le cadre d'une assurance maladie universelle ? Quels sont les obstacles potentiels à un tel projet ? Y a-t-il des mesures pour contourner ces difficultés ? Et quel rôle peut jouer l'Etat dans la réalisation d'une telle oeuvre ?
