Objectif: réaliser un état des lieux des pratiques de la prise en charge du cancer du sein de petite taille non métastasé au CHU de Fort de France, à la MFME en 2008, à partir d'une étude rétrospective exhaustive de 80 cas en étudiant les délais des différentes étapes. Methodes: la population étudiée était constituée de patientes ayant un cancer du sein primitif et primo traité par chirurgie. Les données ont été recueillies dans les dossiers médicaux et au cours d'un entretien avec les patientes. Résultats: la médiane des délais du premier contact avec le chirurgien était de 6 jours. 92,5% des patientes ont été opérées dans un délai de 28 jours et 77 ,5% dans un délai de 21 jours. 68, 8% ont eu une consultation post opératoire dans les 14 jours suivant la RCP postchirurgicale. 6,3%ont eu la chimiothérapie dans un délai de 30 jours suivant la chirurgie et 4,7% ont eu la radiothérapie dans les 56 jours (sans chimiothérapie). 32,4% ont débuté leur premier traitement dans les 42 jours. 97, 5% ont eu une RCP post chirurgicale conforme dont 18 ,8. % ayant eu lieu dans les 14 jours après l'intervention .Aucune patiente n'a eu de dispositif d'annonce avant la chirurgie. 82,5% des dossiers ont une traçabilité des éléments diagnostics et pronostics. 87,5% des patientes ont jugé excellente leur prise en charge organisationnelle. Discussion: Les résultats les plus mauvais concernent les délais de prise en charge post chirurgicale, pour les améliorer plusieurs pistes: - un raccourcissement des délais de compte rendu des examens anatomo-pathologiques impliquant les techniciens de laboratoire, les médecins pas assez nombreux et les secrétaires - un raccourcissement des délais de rendez vous à l'hôpital Clarac une fois le protocole thérapeutique établi et validé par la RCP, impliquant au 1 er chef les secrétaires puis les médecins mais aussi le plateau technique et sa maintenance Conclusion: les délais de prise en charge des patientes ayant un cancer du sein devraient être améliorés pour correspondre aux recommandations actuelles.

Objectifs de l'étude : à travers les données de la littérature, observer la prise en charge obstétricale et oncologique d'une femme chez laquelle un cancer du sein infiltrant a été découvert soit pendant la grossesse, soit dans la première année du post-partum. - Patientes et méthodes : enquête rétrospective réalisée sur dossiers obstétricaux et oncologiques. Étude menée à propos de 2 patientes chez lesquelles un cancer du sein a été découvert en association avec la grossesse et dont l'accouchement a eu lieu à la maternité du pavillon “ La Mère et l'Enfant” du CHU de Besançon entre le 1er janvier 1994 et le 31 décembre 2003. - Résultats et analyse : Le cancer a été découvert chez les deux patientes de notre étude par autopalpation. La grossesse n'aggrave pas le pronostic du cancer du sein, mais le retard au diagnostic fréquemment observé tend à augmenter la fréquence des formes avancées. Le traitement du cancer est discuté selon l'évolution du cancer, le terme de la grossesse et le désir du couple à poursuivre celle-ci. Les grossesses après traitement n'entraînent pas plus de récidives. - Conclusion : Une prise en charge multidisciplinaire est nécessaire afin de prendre en compte tous les paramètres : le cancer, la grossesse, la mère et l'enfant. Une contraception doit être envisagée lors du traitement du cancer.

They work tirelessly, collecting invaluable data in the field under conditions that are always challenging, and at times dangerous.
