L'approche sur-technicisée instaurée durant le siècle passé, à impacté la profession sage-femme. Suite au récent mouvement physiologique, l'objectif de cette étude était de réaliser un état des lieux des connaissances des sages-femmes concernant la prise en charge physiologique de l'accouchement et d'identifier les facteurs pouvant influencer leur capacité à la réaliser de façon pleine et sécuritaire. METHODES : Nous avons réalisé une étude descriptive observationnelle auprès des sagesfemmes de la région Grand Est de novembre 2018 à février 2019. Ont été exclues ou non inclues les sages-femmes libérales n'exerçant pas en maison de naissance, les sages-femmes de PMI et enseignantes et les sages-femmes ne réalisant pas d'accouchements. Les hypothèses émises étaient que l'approche actuelle de la parturition ne permettait pas de garantir pleinement le respect du rythme et de la physiologie du travail, était marquée par un manque de connaissances sur les techniques non médicamenteuses de prise en charge de la douleur et ne permettait pas toujours aux patientes d'être actrices de leur accouchement. De plus, il était émis que les sages-femmes sont freinées dans leur prise en charge par un manque de formation et de temps. RESULTATS : 101 sages-femmes ont été incluses, 2 exclues. Elles sont freinées dans ce type de prise en charge par un manque de formation et de temps. Pourtant, elles sont 59,4% à se sentir capable et à réaliser une prise en charge physiologique. Elles sont 47,5% à être formé à une technique de prise en charge non médicamenteuse de la douleur. DISCUSSION : Les connaissances des modalités d'exécution sont mitigées. La méconnaissance des critères d'exclusion est un problème puisque cette prise en charge prône la sécurité. La détechnicisation est un concept intégré, contrairement au respect de la physiologie du travail et de l'approche du soin relationnel.

These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children.
