Introduction : En France, 13,5% des décès ont lieu en EHPAD, la fin de vie y est devenue un enjeu de plus en plus important et nécessite un accompagnement spécifique. L'objectif principal de cette étude était d'analyser la prise en charge de la fin de vie en EHPAD hospitalier, puis secondairement, de comparer ces résultats à ceux de l'étude nationale de 2013 « La fin de vie en EHPAD ». Méthodologie : Cette étude rétrospective a été menée au sein de l'EHPAD hospitalier du CHU de Poitiers, qui comprend 164 résidents. Tous les dossiers de résidents décédés du 01/01/2017 au 31/12/2017 ont été analysés. Pour chacun d'eux, des données sociales cliniques, médicamenteuses, mais aussi le déroulement de la fin de vie sur les quinze derniers jours, ont été répertoriées puis anonymisées. Résultats : 35 dossiers ont été recensés avec un âge moyen de 90,2 ans, 60% de femmes. Les données de 31 résidents ont été analysées car 4 résidents décédés avaient été transférés à l'hôpital. Soixante-huit pour cent ont eu accès à une chambre seule. Dans les quinze derniers jours, 77% ont eu une prescription de limitation thérapeutique, 81% ont reçu des antalgiques de palier III. Dans la dernière semaine 84% des résidents avaient des douleurs très bien soulagées, 26% étaient dans un réel inconfort physique, 77% étaient entourés de leurs proches. Dans les dernières 24 heures, 68% ont reçu un traitement par benzodiazépines et 77% ont eu un décès jugé paisible. Discussion : A notre connaissance, aucune étude ne s'est intéressée aux spécificités de la fin de vie en EHPAD hospitalier. En comparaison avec les résultats de l'étude nationale de 2013, notre étude montre une fin de vie plus médicalisée, des prescriptions de limitation thérapeutique plus fréquentes, l'utilisation d'antalgiques de palier III et de traitements à visée sédative dans les dernières heures, et une baisse du nombre des transferts vers le service des urgences. Conclusion : La prise en charge de la douleur et le confort du résident apparaissent être des points forts dans notre EHPAD hospitalier. Il serait intéressant de poursuivre ce travail dans d'autres EHPAD hospitaliers afin de confirmer ces résultats car ils ouvrent la voie à une réflexion sur la médicalisation de la fin de vie en EHPAD.
