Les interventions non médicamenteuses (INM) dans la maladie d'Alzheimer suscitent un intérêt croissant, dans un contexte actuel de thérapeutiques médicamenteuses incertaines. Après avoir effectué une revue de la littérature à ce sujet, nous avons interrogé des médecins traitants en vue d'établir un état de lieux de la prescription des INM, de rechercher les critères influençant leur prescription et d'évaluer les difficultés de coordination des soins. Enquête de pratique et d'opinion prospective descriptive auprès de 75 médecins généralistes de la région Rhône-Alpes. Les INM sont encore insuffisamment validées, devant les faiblesses méthodologiques des études. Cependant ce manque de validation est de moins en moins incertain, et nous avons essayé de dégager des critères d'efficacité des INM. Notre enquête a permis d'interroger les médecins sur leur dernier patient Alzheimer au stade léger, par rapport à leurs prescriptions de 5 groupes d'INM portant sur la cognition, l'affect et l'émotion, le comportement et l'environnement, l'activité physique et la communication. Les résultats montrent que les médecins sont peu prescripteurs d'INM, malgré leur motivation et fait qu'ils pensent qu'elles sont indiquées et efficaces. De nombreux obstacles limitent cette prescription : un système de soin basé sur la pharmacologie, le caractère hétérogène des INM, une prise en charge globale insuffisante de la part des médecins généralistes, un manque d'intervenants et de structures, des difficultés de coordination des soins.
