Autant l'utilisation des morphiniques dans les douleurs cancéreuses est reconnue et bien codifiée, autant leur utilisation dans les douleurs chroniques d'origine non-cancéreuses, bien qu'ayant récemment augmenté, reste encore sujette à caution. Cependant les médecins, les prestataires de soins, les législateurs, se demandent mutuellement conseil ou recherchent un consensus sur l'utilisation appropriée des opioïdes forts dans les douleurs chroniques non-cancéreuses. - Notre travail de recherche est une étude rétrospective portant sur 55 patients, âgés de 17 à 101 ans, ayant consulté les centres anti-douleur de Lons-le-Saunier et de Besançon au cours de l'année 2004, pour la prise en charge de douleurs chroniques non-cancéreuses et ayant bénéficié d'un traitement par opioïdes forts. En France, la prévalence de la douleur chronique est estimée à 31,7 %, soit près de 14 millions de Français concernés. La prise en charge de la douleur est une priorité de santé publique, notamment avec les trois plans nationaux de lutte contre la douleur successifs de 1998 à 2010. Cependant, 44 % de ces personnes rapportent une prise en charge inadaptée de leur douleur et 64 % des patients recevant un traitement, rapportent n'être que modérément soulagés sur le long terme. Bien que le traitement par opioïdes forts pour des douleurs d'intensité modérée à forte soit admis pour les pathologies cancéreuses, l'utilisation de ce type de traitement reste plus controversée pour les pathologies chroniques non cancéreuses. Notre étude se propose d'évaluer les habitudes de prescription des médecins enseignants cliniciens ambulatoires de la Gironde dans les douleurs chroniques non cancéreuses. Les résultats de notre étude semblent encourageants : même si la prescription des morphiniques est mal encadrée en France, les médecins généralistes enseignants cliniciens ambulatoires de la Gironde semblent bien prescrire ce traitement pour les douleurs chroniques non cancéreuses. De nombreux freins persistent encore même s'ils sont parfois injustifiés ou contrôlables (effets secondaires, représentation de la morphine, peur d'une addiction...). Il n'en demeure pas moins que de nombreux efforts restent à faire en ce qui concerne la formation initiale et continue des médecins, l'élaboration de meilleuires recommandations et le développement de la coopération interprofessionnelle pour une meilleure prise en charge de la douleur.
