Les thérapies ciblées orales contribuent à la mise en place d'un nouveau parcours de soin. En raison de la marge thérapeutique étroite de ces anticancéreux, les patients sont exposés à des risques iatrogènes importants dont les interactions médicamenteuses. Cela représente un enjeu de santé publique nécessitant l'implication du pharmacien. L'auteur a analysé les résultats des pratiques du pharmacien en matière d'accompagnement du patient sous thérapie ciblée orale au sein du Centre Jean Perrin. L'objectif était d'évaluer l'implication du pharmacien dans la détection et la prévention des interactions médicamenteuses. Pour ce faire, le pharmacien participe aux consultations tripartites dans lequel il réalise une consultation pharmaceutique. Au travers de son analyse pharmaceutique, son entretien avec le patient et l'instauration du lien ville-hôpital, il oeuvre pour la prévention des interactions médicamenteuses, l'efficacité et la sécurité des thérapeutiques. Sur un total de 222 consultations, 111 interventions pharmaceutiques ont été recensées. Parmi les interactions détectables, 75% concernent des médicaments, 14% l'alimentation et 11% les médecines alternatives et complémentaires. Lors de la mise en évidence d'interactions médicamenteuses, le pharmacien proposera au médecin traitant différentes alternatives : l'arrêt du traitement, une adaptation posologique, un suivi ou une alternative thérapeutique, une information concernant les modalités d'administration... Notre étude met en évidence le rôle essentiel dans la détection et la prévention des interactions médicamenteuses.
