Contexte : L'ultrafiltration par dialyse péritonéale (DP) est utilisée avec succès pour traiter les patients atteints d'insuffisance cardiaque chronique réfractaire (ICCR) dans un contexte de syndrome cardio-rénal de type 2 ou 4. L'étude est réalisée dans le but d'évaluer la mortalité et le devenir des patients traités par ultrafiltration péritonéale. Patients et méthodes : Il s'agit d'une étude rétrospective incluant les patients qui ont requis l'indication d'ultrafiltration par DP pour ICCR entre janvier 1995 et décembre 2010 dans 2 centres de France. Les marqueurs de l'insuffisance cardiaque comme le poids, la posologie de diurétique, les hospitalisations pour insuffisance cardiaque aiguë, et la fraction d'éjection du ventricule gauche (FEVG, %) en échocardiographie étaient relevés à l'inclusion et pendant la première année. La durée de la DP, le nombre de péritonites et les décès ont été également analysés. Résultats : Sur 749 patients en DP sur cette période, 126 (17 %) ont été inclus (âge moyen, 72 + ou - 11 ans). Le débit de filtration glomérulaire estimé moyen avant le début de la DP était de 33,5 + ou -15,1 mL/min/1,73 m2. Durant la première année de DP, le nombre de jours d'hospitalisation pour insuffisance cardiaque aiguë diminue de façon spectaculaire après la mise en DP (3,3 +ou - 2,6 vs. 0,3 + ou - 0,5 jour/patient/mois ; p

L’atteinte rénale au cours de l’insuffisance cardiaque chronique, ou syndrome cardio-rénal est responsable de surcharge hydrosodée avec résistance aux diurétiques, altérant le pronostic et la qualité de vie. La dialyse péritonéale (DP) a été proposée comme traitement des syndromes cardio-rénaux mais il n’existe que très peu d’études dans la littérature, de petites cohortes, pour la plupart antérieures aux avancées thérapeutiques cardiologiques récentes, et souvent chez des patients au stade réfractaire de leur maladie cardiaque. Méthodes. Etude rétrospective multicentrique régionale (Nord-Pas-de-Calais) de 2000 à 2007 comparant dyspnée (stade NYHA) et hospitalisations pour décompensation cardiaque avant et après un an de DP instaurée pour syndrome cardio-rénal chez des patients insuffisants cardiaque sous traitement médical bien conduit. Résultats. 31 patients présentaient les critères d’inclusion (âge moyen 69,7 +/- 10,2, en déviation standard), 24 (77%) avaient une dyspnée au stade III de la NYHA, 7 (31%) au stade IV. Le débit de filtration glomérulaire moyen était de 33,9 +/- 10,3 ml/min/1,73m2 (formule du MDRD simplifié).
