Introduction : La maltraitance infantile intrafamiliale est un sérieux problème de santé publique, et engendre des répercussions importantes sur la santé globale de l'enfant et sur son avenir. La prévention est primordiale et débute par le repérage des situations à risque. Objectif : Pondérer les facteurs de risque (FDR) identifiés afin d'élaborer un score de repérage d'une situation à risque de maltraitance intrafamiliale chez le mineur en médecine générale. Méthode : Un consensus formalisé d'experts par rondes Delphi a été conduit entre novembre 2019 et avril 2020. Les experts inclus travaillent en lien avec la protection de l'enfance. 25 facteurs de risque proposés étaient tirés d'une revue systématique de la littérature réalisée en 2016. Une échelle de risque de 1 à 5 points standardisée a été utilisée. Résultats : Trois rondes Delphi ont été réalisées incluant 40 experts. Les 25 FDR proposés ont obtenu une pondération consensuelle permettant l'élaboration d'un score de repérage d'une situation à risque de maltraitance intrafamiliale chez le mineur à destination du médecin généraliste. Aucun désaccord n'a été notifié. Les FDR ayant eu le plus haut score à 5 points sont la violence conjugale, les troubles de l'attachement parent-enfant, les antécédents de maltraitance chez l'enfant, la fratrie ou les parents, et l'abus d'alcool ou la consommation de drogues chez les parents. Conclusion : En outre, les commentaires des experts ont permis de recentrer le score dans le contexte global en précisant l'importance d'une association de FDR pour le repérage d'une situation à risque, l'existence de la maltraitance dans tous les milieux socio-économiques, et le rôle des troubles trans-générationnels du lien parent-enfant. Des études sont nécessaires pour évaluer l'applicabilité de ce score en consultation de médecine générale et son aide au repérage.

Les maltraitances sexuelles chez le mineur en milieu intrafamilial sont des situations fréquentes, graves, mais sous évaluées en raison d'un manque de connaissance dans le repérage. Le médecin généraliste a un rôle central à tenir afin de repérer précocement les situations à risque. 58 dossiers du tribunal de grande instance de Toulouse, portant sur des cas d'inceste sur mineurs, ont été analysés. Les filles sont plus souvent agressées sexuellement que les garçons (respectivement 69% et 31%). Les enfants les plus exposés sont âgés de 7 à 12 ans. Les agresseurs sont dans 95% des cas de sexe masculin et sont en majorité le beau-père ou le père. Un quart des mères des victimes ont elles-mêmes subi cette violence dans leur enfance. Près d'une sur deux a eu son premier enfant à 20 ans ou moins. La violence conjugale et la violence physique sur l'enfant se constatent dans respectivement 47% et 43% des situations. L'alcoolisme est retrouvé chez 41% des agresseurs majeurs. Les familles recomposées avec beaucoup d'enfants sont plus touchées. Dans l'ensemble des familles, les aînés et les seconds sont plus souvent les victimes. 43% des familles ont été repérées en danger préalablement aux abus sexuels. Le médecin généraliste doit s'informer et se former à rechercher et repérer ces situations à risque en se basant sur des recommandations validées.

Rethinking Domestic Violence is the third in a series of books by Donald Dutton critically reviewing research in the area of intimate partner violence (IPV). The research crosses disciplinary lines, including social and clinical psychology, sociology, psychiatry, affective neuropsychology, criminology, and criminal justice research. Since the area of IPV is so heavily politicized, Dutton tries to steer through conflicting claims by assessing the best research methodology. As a result, he comes to some very new conclusions. These conclusions include the finding that IPV is better predicted by psychological rather than social-structural factors, particularly in cultures where there is relative gender equality. Dutton argues that personality disorders in either gender account for better data on IPV. His findings also contradict earlier views among researchers and policy makers that IPV is essentially perpetrated by males in all societies. Numerous studies are reviewed in arriving at these conclusions, many of which employ new and superior methodologies than were available previously. After twenty years of viewing IPV as generated by gender and focusing on a punitive "law and order" approach, Dutton argues that this approach must be more varied and flexible.
