How did seven low- and middle-income countries, inspired by the landmark Alma-Ata Declaration, dramatically improve citizen health by focusing on primary health care? The Alma-Ata Declaration of 1978 marked a potential turning point in global health, signaling a commitment to primary health care that could have improved the safety of air, food, water, roads, homes, and workplaces in all 180 countries that signed it. Unfortunately, progress in many countries stalled in the 1980s. The declaration was, however, embraced by a number of countries, where its implementation led to substantial improvement in citizen health. Achieving Health for All reveals how, inspired by Alma-Ata, the governments of seven countries executed comprehensive primary health care systems, deploying new cadres of community-based health workers to bring relevant services to ordinary households. Drawing on a set of narrative case studies from Bangladesh, Indonesia, Ethiopia, Nepal, Ghana, Sri Lanka, and Vietnam,the book explains how a primary health care focus succeeded in improving population health. The book also conclusively demonstrates that comprehensive, multisector, community-controlled, and population-level primary health care is a viable strategy that, against the odds, has led to sustainable, scalable good health at lower cost. Bangladesh is committed to achieving universal health coverage (UHC) by 2032; to this end, the government of Bangladesh is exploring policy options to increase fiscal space for health and expand coverage while improving service quality and availability. Despite Bangladesh’s impressive strides in improving its economic and social development outcomes, the government still confronts health financing and service delivery challenges. In its review of the health system, this study highlights the limited fiscal space for implementing UHC in Bangladesh, particularly given low public spending for health and high out-of-pocket expenditure. The crisis in the country’s human resources for health (HRH) compounds public health service delivery inefficiencies. As the government explores options to finance its UHC plan, it must recognize that reform of its service delivery system with particular focus on HRH has to be the centerpiece of any policy initiative.

Unannounced visits were made to health clinics in Bangladesh to determine what proportion of medical professionals were at their assigned post. Averaged over all job categories and types of facility, the absentee rate was 35 percent. The absentee rate for physicians was 40 percent at the larger clinics and 74 percent at the smaller sub-centers with a single physician. Whether the medical provider lives near the health facility, the opportunity cost of the provider's time, road access, and rural electrification are highly correlated with the rate and pattern of absenteeism.

Sylhet, the area of Bangladesh most closely associated with overseas migration, has seen an increase in remittances sent home from abroad, introducing new inequalities. Social change has also been mediated by the global forces of Western biomedicine and orthodox Islam. This book examines the effects of these modernizing trends on mental health and on local, traditional healing as the new inequalities have exacerbated existing social tensions and led to increased vulnerability to mental illness. It is the young women of Sylhet who are most affected. The global economy has increased competition for resources and led to marriage being seen as a route to economic advancement. Parents prefer to give their daughters in marriage to families that will widen their social contacts and enhance their economic and social standing. Accordingly, the young wife's outsider status (and hence vulnerability to mental illness) has increased as it is no longer customary to give daughters in marriage to local kin. Yet, patients and their families do not work out tensions passively. They are active agents in the construction of their own diagnosis. The extent to which patients act or are acted upon is an investigation that runs throughout the book. Alyson Callan is a psychiatrist and anthropologist.
