This paper reviews the performance of the health sector in Uganda. It addresses concerns in the Ugandan health community that health financing must be increased to improve health, nutrition, and population outcomes, especially given the rapid increase in the country s population. Although international development aid targeted to health has increased dramatically, Uganda s first priority is actions to reduce waste and inefficiency in existing health expenditures. Such actions could include improved management of human resources in the health sector, strengthened procurement and logistics management for medicines and medical supplies, and better programming of development assistance of health. This paper is targeted to health policy makers and those involved in health services financing, both in the government and in donor agencies. This working paper was produced as part of the World Bank s Africa Region Health Systems for Outcomes (HSO) Program. The Program, funded by the World Bank, the Government of Norway, the Government of the United Kingdom, and the Global Alliance for Vaccines and Immunization (GAVI), focuses on strengthening health systems in Africa to reach the poor and achieve tangible results related to Health, Nutrition, and Population. Untreated mental illnesses impair the social and economic lives of affected individuals significantly. In the eyes of the public in Uganda any individual who suffers from mental illness is not expected to regain his/her normal senses and return to normality. Contrary to this widely held belief, however, people have adequately recovered from severe mental illness and regained good productive lives after receiving proper treatment. This book has been written to foster quality health care and a good quality of life for the mentally ill in Uganda. The government of Uganda has fully integrated mental health in the national minimum health care package at all health units; with the result that every health care provider will care for one mentally ill patient at least once a month. The requirements of both the mental health care provider and the general health care provider in Uganda are covered. Emilio Ovuga is Professor of Psychiatry at Makerere University. In the mid-1800s, a group of High Anglicans formed the Universities' Mission to Central Africa (UMCA). Inspired by Dr. David Livingstone, they felt a special calling to bring the Church, education, and medical care to rural Africans. To deliver services across a huge, remote area, the UMCA relied on steamer ships that were sent from England and then reassembled on Lake Malawi. "The government of Uganda faces a multitude of challenges in the health care arena, from ensuring that health care services are delivered in the most equitable manner, to structuring the health care delivery system to be most effective, to waging campaigns against the leading causes of mortality and morbidity. The government needs the best information available on existing conditions, in order to devise strategies to successfully meet these challenges." This technical paper summarizes the results of three research efforts concerning current health care issues in Uganda. Its purpose is to summarize key actions and identify new challenges. In Part I, "Equity of Access to Health Services in Uganda: The Effects of Income, Gender, Proximity to Services, and Quality of Care," the author examines a number of factors that affect access to health services, including quality of services and income. Part II of this technical paper, "Decentralization of Health Services in Uganda: Moving Toward Improved Delivery of Services," summarizes the key actions that have been undertaken during the decentralization of Uganda's health sector and identifies challenges that are emerging during this process. Part III, "Malaria: A Priority Health Problem," summarizes the status of malaria control in Uganda and highlights some of the key issues that need to be addressed to further strengthen malaria control efforts.
