Introduction : l'objectif de ce travail est d'analyser les freins au dépistage du cancer colorectal pour les personnes en situation de précarité. Plusieurs travaux avaient déjà exploré les freins à ce dépistage mais pas dans cette situation spécifique. L'enjeu est important pour réduire les inégalités sociales de santé dont l'accès à la prévention est un axe essentiel. Méthode : il s'agit d'un travail qualitatif effectué entre juin et septembre 2019 avec interview de 16 personnes recrutées en amont du système de soins, aux Restos du Cœur d'Ivry. Les interviews ont été enregistrées puis retranscrites anonymement. Un travail de codage ouvert puis de codage axial a été réalisé afin de dégager les freins. Résultat : Des résultats non attendus sont apparus. Premièrement, les personnes recrutées avaient un taux de dépistage comparable à la population générale sur cet échantillon et secondairement, 85% avaient un médecin traitant. Nous expliquerons ces faits inattendus. Cependant, des freins spécifiques ont été trouvés : le concept même de dépistage n'était pas connu, le courrier non compris ou non lu, des freins liés au système de soins (modernisation de la prise de rendez-vous sur internet, consultations réalisées dans l'immédiateté et saturées par d'autres objectifs, absence de proposition du médecin traitant..), des freins liés au patient (préoccupations autres, personnalités, convictions personnelles, superstitions....), des freins liés à la société (aspect culturel, méconnaissance du système...), freins liés à la précarité (isolement social, financier, administratif, insécurité, priorisation des problèmes...).Les effets d'un temps autour de ce sujet a conduit à la demande du test par quatre des personnes interrogées. Discussion : ce travail présente un certain nombre de forces et de limites du fait de son schéma. Les freins connus ont été retrouvés et, de nouveaux ont pu être repérés. Un travail sur la littérature existante a permis d'envisager des voies d'amélioration afin de favoriser le dépistage du cancer colorectal et diminuer les inégalités sociales de santé.
