Contexte : 26% des femmes françaises utilisent un dispositif intra-utérin comme moyen de contraception alors que cette méthode est l'une des plus efficaces. En tenant compte du fait que plus d'un tiers des femmes consultent un médecin généraliste pour la prescription et le suivi de leur contraception, le rôle du médecin généraliste dans la prévention des grossesses non désirées est important. Objectifs : Déterminer si avec une « bonne formation » les médecins généralistes ne posant pas de DIU envisageraient d'en poser et préciser alors les besoins en formation. Méthode : Analyse des pratiques des médecins généralistes de 6 cantons de Vendée (85) concernant les DIU et notamment leurs besoins en formation par une enquête postale sous forme d'un questionnaire. Résultats : Le manque de formation n'est pas le seul obstacle à la pose de stérilets en cabinet libéral de médecine générale ; on constate que seulement 42% des médecins généralistes ne posant pas de DIU accepteraient d'en poser après une « bonne formation ». Se former auprès d'un confrère gynécologue à ce geste technique leur semble être la manière la plus adaptée. Conclusion : Aider les médecins généralistes à mieux se former sur les différents moyens de contraception ne semble pas suffisant pour permettre aux femmes d'accéder à un plus large choix de méthodes contraceptives ; d'autres paramètres sont à prendre en compte.

La pose de dispositif intra-utérin est un geste technique ouvert aux médecins généralistes, sage-femme et gynécologues. Le dispositif intra-utérin est une contraception simple, très efficace et accessible à une majorité de femmes quelle que soit leur parité dont la formation à la pose entraine une augmentation de son utilisation. L'objectif de ce travail est de créer un outil numérique didactique d'aide à la pose de dispositif intra-utérin. Une revue narrative de la littérature à été réalisée sur différents éléments clés : l'asepsie et le risque infectieux, la technique de pose, le matériel nécessaire, les effets secondaires et les traitements associés. Les indications, contre-indications et le suivi ont fait l'objet d'analyse de pratiques et de recommandations officielles. Enfin les critères de qualité vidéo ont été actualisés à l'appui de thèse similaires antérieures. Nos résultats montrent que le risque infectieux est appréhendé par un algorithme sans que la pose ne soit retardé, il existe deux techniques différentes de pose, les dispositifs étudiés et choisis sont le cuivre à 380mm et le levonorgestrel 52mg posés avec un kit de pose pré-inclus avec le dispositif intra-utérin. Les effets secondaires sont rares, le Naproxène 550mg ou le Tramadol 50 mg peuvent être proposés comme traitement antalgique. Ces résultats ont permis de créer une vidéo didactique complète, pratique et de durée courte diffusée sur une plateforme publique après validation des pairs. Ce travail contribue à encourager les professionnels de santé à la réalisation de geste technique en cabinet.

Le dispositif intra-utérin (DIU) est une contraception de longue durée d'action très efficace destinée à toutes les femmes quelle que soit leur parité. Il persiste cependant des freins au choix de cette méthode dont la forte anticipation de la douleur liée à l'insertion du DIU. Améliorer le confort des poses de DIU pourrait permettre de diminuer ce frein. Mettre en évidence les conditions favorables à un vécu positif de la pose du DIU dans le cabinet de médecine générale. Une étude qualitative par entretiens semi-dirigés a été menée auprès de 15 patientes recrutées auprès de 6 médecins généralistes ayant inséré le DIU. Résultats et Discussion. La vision globale de la mise en place d'un DIU a permis de mettre en évidence différents facteurs intervenant dans le vécu, avant, pendant et après le geste. Des explications claires, un contexte favorable, une patiente bien installée, un médecin bienveillant, accessible et compétent, une technique d'insertion et une prescription d'antalgiques réfléchie, une consultation de suivi rassurante, l'absence de complication et une satisfaction globale du DIU et de sa pose ont contribué à un vécu positif. A l'inverse les appréhensions, la gêne liée à l'intimité, les douleurs et les saignements ont contribué à un vécu négatif de la pose. L'importance d'une relation médecin-patiente de qualité et de confiance lors de la pose du DIU prédomine sur l'acte. La prise en compte du ressenti des patientes est indispensable pour faciliter le vécu positif de ce geste.

Most women who die from cervical cancer, particularly in developing countries, are in the prime of their life. They may be raising children, caring for their family, and contributing to the social and economic life of their town or village. Their death is both a personal tragedy, and a sad and unnecessary loss to their family and their community. In addition, because family planning programmes often significantly affect population growth, the study examines the impacts of family planning on fertility and health, and the human rights implications of family planning programmes. In considering the book's conclusions about the impact of population growth on development, four caveats should be noted. First, the effects of population growth vary from place to place and over time. Thus, blanket statements about overall effects often cannot be made. Where possible, the authors note the contexts in which population effects are strongest and weakest. Second, all of the outcomes examined in this book are influenced by factors other than population growth. Thoroughly revised and updated, the new edition further refines the WHO method, which advocates the use of a small number of relatively inexpensive drugs, including morphine. Revisions draw on experiences with millions of patients around the world as well as new knowledge about the specific pain syndromes unique to cancer. Completely new are chapters describing the international system by which morphine and other opioids are made available for medical purposes. The book has two parts. Part one provides a practical guide to the relief of cancer pain, concentrating on drug treatment as the mainstay of pain management. The most extensive section sets out detailed guidelines for the selection and prescribing of non-opioid analgesics, opioid analgesics, drugs for neuropathic pain, and adjuvant drugs for the treatment of adverse effects, the enhancement of pain relief and the management of concomitant psychological disturbances.
