The cuts imposed on the FCO since 2010 have been severe and have gone beyond just trimming fat: capacity now appears to be being damaged. If further cuts are imposed, the UK's diplomatic imprint and influence would probably reduce, and the Government would need to roll back some of its foreign policy objectives. The FCO's budget is a tiny element of Government expenditure, but the FCO makes disproportionate contribution to policy making at the highest level, including decisions on whether to commit to military action. The next Government needs to protect future FCO budgets under the next Spending Review.

15 million NHS patients in England with long-term conditions such as diabetes, arthritis and asthma account for 70% of the annual expenditure of the NHS in England. One projection estimating that the bill for treatment of long-term conditions will require the NHS to find £4 billion more each year by 2016. Increasingly, patients do not have a single long-term condition but live with two or more conditions, complicating treatment and adding to its cost. The Committee strongly supports the development of individual care planning for people with long-term conditions, based on the principles successfully demonstrated in the NHS House of Care programme. Care planning approaches will involve GPs, community health services and specialists sitting down with the patient to draw up a personalised plan for the care required, which includes the support needed to help the patient manage his or her own condition. The challenge, though, of introducing personalised care planning for 15 million people is substantial. The Committee looked at the prevailing view that services to treat long-term conditions should be moved out of hospitals and into primary and community care. To provide effective care for these conditions, services have to be maintained across all settings, from support in the home through to acute specialist care, and many conditions will continue to require specialist services delivered in hospital. Effective management of long-term conditions also requires collaboration with other government providers, such as housing and transport services.

This reports finds that the Coalition Government has developed in public a more mature and measured relationship with the US, although there has been no fundamental change in the nature of the tie. The Committee declares the relationship to be in good health. In particular, the Committee said that it was not aware of any evidence that the House of Commons vote in August 2013 against potential military action in Syria had damaged the UK's relationship with the US. Rather, the Committee concluded that the episode illustrated general features of the UK-US relationship, namely that developments in the UK could influence US policy; and that the underlying tie was resilient. Today's publication follows up a report produced by the previous Foreign Affairs Committee at the end of the last Parliament, which recommended that the UK Government should adopt a more hard-headed and less deferential approach to the US. The Coalition Government seemed to have taken up this recommendation. The Committee criticises the UK Government's poor provision of information about the UK-US Joint Strategy Board, which was created during President Obama's State Visit to the UK in May 2011. Among strategic issues that it considered, the Committee agreed with the Government that the proposed EU-US Transatlantic Trade and Investment Partnership (TTIP) could have significant positive strategic impact for the UK and the transatlantic relationship.

In 1966, the UK concluded an Agreement with the United States giving it permission to use the British Indian Ocean Territory, including the island of Diego Garcia, for defence purposes for an initial period of 50 years. Unless the UK or the US takes steps to terminate the Agreement, it will automatically be extended in 2016 for a further twenty years. The disclosure in 2008 that the US had, contrary to previous statements by the FCO, used facilities at Diego Garcia in the course of rendition (the practice of sending a foreign criminal or terrorist suspect covertly to be interrogated in a country with less rigorous regulations for the humane treatment of prisoners) since 2001, dented public confidence in the UK's ability to exercise control over its sovereign territory. If the UK allows the 1966 Agreement to be extended beyond 2016, the text should be revised. It should specify that any extraordinary use of the US base or facilities, requires prior approval from the UK Government; and it should state explicitly that the British Indian Ocean Territory should not be used for rendition unless authority has first been granted by the UK Government, on a case by case basis

Emissions of a number of airborne pollutants increased in 2013, after being steady between 2010 and 2012 and in a longer term decline before that. The UK failed to meet targets for nitrogen dioxide pollution in 34 of the 43 zones specified in the EU Ambient Air Quality Directive in 2012, resulting in the European Commission launching infraction proceedings against the UK in February 2014 in regard to 16 zones that would not be compliant by 2015. The Committee's report recommends an overarching Environmental Strategy be implemented, to set out strategic principles and good practices; facilitate discussion between central and local government and identify how they can work together and with the wider community; encompass clear environmental assessments; identify work required to fill data gaps in assessments; map appropriate policy levers to environmental areas; and set out how environmental and equality considerations will be addressed in policy areas across Government. The report concludes that the Government should set up an independent body-an 'Office for Environmental Responsibility'-to (i) review the Environment Strategy we advocate; (ii) advise Government on appropriate targets; (iii) advise Government on policies, both those in Government programmes and new ones that could be brought forward to support the environment; (iv) advise Government about the adequacy of the resources (in both central and local government) made available for delivering the Strategy; and (v) monitor and publish performance against the Strategy and its targets.

England is a litter-ridden country compared to most of Europe, North America and Japan. Levels of litter in England have hardly improved in the past 12 years and the best estimates are that litter costs the taxpayer between £717 and £850 million a year to clear up. Change is needed. There has been a 20% increase in fast-food litter in the last year. The Government should bring forward legislation requiring all shops, restaurants and retail food outlets to keep the perimeters of their premises free from litter. Responsible businesses are already doing this. In addition, the fast-food industry should introduce 'on-pack' information on all branded take-away and fast-food packaging to remind consumers to dispose of litter responsibly. The most frequently littered items are chewing gum and smokers' materials. Chewing gum and staining are difficult and costly to remove. This was a matter of considerable concern upon which the Committee deliberated at length. Levels of fly-tipping increased by 20% in the last year. There were 852,000 reported incidents but only 2,000 convictions in the courts. The Government should introduce a fixed penalty notice for fly-tipping for household items - the bulk of the incidents - and the industry must introduce a scheme to take away unwanted household appliances and furniture when replacements are delivered. In the end it is individuals who litter and fly-tip their unwanted goods, and it is this behaviour which needs to change. The Committee support a variety of behaviour-changing activities and campaigns to prevent littering.

GMC's fitness to practise successfully produces outcomes that protect patients from sub-standard doctors but failures to communicate the reasons for decisions and poor investigative practices have undermined a small number on investigations. The GMC should review its fitness to practice procedures to prevent such mistake. The Committee also found that while it is still too early to judge whether revalidation has been effective there is a worrying approach to the oversight of revalidation. Each designated body has a responsible officer for revalidating their medical staff, but the degree to which the responsible officer will be held to account is unclear. It is imperative that the GMC clarifies the personal responsibility and accountability of responsible officers. There is also concern over the number of responsible officers available to oversee the revalidation of doctors working in primary care. GPs are revalidated not by their own employers but by one of the 27 NHS England local area teams that oversees Clinical Commissioning Groups in England. Just 27 responsible officers will be tasked with overseeing revalidation for approximately 45,0000 GPs in England. The Government's intention had been to give the GMC the power to allow it to appeal decisions made by the Medical Practitioners Tribunal Service (MPTS) in 2014 by using a mechanism in secondary legislation called a section 60 order. The Government now plans to introduce the reform in primary legislation as part of a proposed Law Commission Bill thus meeting with even further delay
