Introduction : Le pronostic des patients victimes d'un accident vasculaire cérébral (AVC) admis en réanimation est classiquement considéré comme péjoratif. L'objectif de notre étude était d'évaluer la mortalité, le pronostic fonctionnel et les facteurs pronostiques des AVC admis en réanimation. Matériel et méthodes : Nous avons réalisé une étude monocentrique rétrospective observationnelle portant sur des adultes admis en réanimation au centre hospitalier universitaire (CHU) de Besançon du 1er janvier 2015 au 31 décembre 2017 pour prise en charge d'un infarctus cérébral (IC) ou d'un hématome intra-parenchymateux (HIP). Le pronostic fonctionnel et la mortalité ont été évalués par le score de Rankin modifié (mRS) en fin d'hospitalisation et entre 3 et 6 mois. Une évolution favorable était définie par un mRS 3. Résultats: Cent cinquante six AVC ont été inclus: 101 IC (64,7%) et 55 HIP (35,3%). La moyenne d'âge était de 66,8 ans (32-90 ans), 56,4% des patients étaient des hommes, et l'autonomie préalable était de 96,8%. La mortalité hospitalière des AVC, IC et HIP était de 48,1, 40,6 et 61,8%, et la mortalité entre 3 et 6 mois était de 57,5, 52,7 et 66%. Parmi les survivants, le taux d'AVC, IC et HIP ayant un mRS 3 était de 21, 20 et 23,8% en intra-hospitalier, et de 50,8, 52,3, et 47,1% entre 3 et 6 mois. Le mRS médian des survivants était à 4 (IQR 4-5) en fin d'hospitalisation, et à 3 (IQR 2-4) entre 3 et 6 mois (3 pour les IC et 4 pour les HIP, sans différence significative). Les facteurs significativement associés à un mauvais pronostic à moyen terme étaient pour les IC l'hypertension artérielle, un volume lésionnel élevé et notamment supérieur à 145 ml, un engagement sous falcoriel d'emblée, et la majoration du volume, un œdème cérébral, un engagement sous falcoriel ou temporal au contrôle; et pour les HIP un âge élevé, une admission pour coma et une localisation profonde.Conclusion: Le pronostic des AVC admis en réanimation reste péjoratif et la mortalité hospitalière élevée, mais le pronostic fonctionnel à moyen terme n'est pas majoritairement défavorable.
