Les facteurs de risque cardiovasculaire sont actuellement la première cause de morbimortalité dans les pays industrialisés, ils représentent donc un important problème de santé publique. Les études de prévalence des facteurs de risque cardiovasculaire, de leurs associations et de leurs prises en charge sont peu nombreuses dans les populations des Départements d'Outre Mer. C'est pourquoi il nous semble intéressant d'apprécier, sur un échantillon de population guadeloupéenne, la prévalence de chacun des facteurs de risque tout en proposant une prise en charge aux personnes dépistées. Cette étude s'appuie sur l'examen de santé de 7385 sujets , âgés de 18 ans ou plus, effectué dans le Centre de Santé de Sainte Geneviève, référent en Guadeloupe. L'évaluation concerne le dépistage de l'hypertension artérielle, de l'obésité, des dyslipidémies, de la sédentarité, du tabagisme et du diabète. Dans cette population, la consommation de tabac est faible et à prédominance masculine ; 50,4 % des femmes et 33,9 % des hommes sont sédentaires ; 10,7 % des hommes et 25,7 % des femmes ont un Indice de Masse Corporelle supérieur ou égal à 30 kg/mø, l'obésité sévère et massive est à prédominance féminine, ainsi que l'obésité abdominale ; il existe une glycémie > 7 mmol/l ou un diabète chez 5,8% des hommes et 7,5% des femmes et 61 % de ces sujets se savaient diabétique ; une dyslipidémie est retrouvée chez 18,5% des hommes et 15,6 % des femmes, parmi eux 23,6 % se connaissaient comme ayant une dyslipidémie et 63,6 % avaient des taux sériques de cholestérol total et de triglycérides normaux. ; la prévalence d'une Pression Artérielle (PA)>140/90 mmHg est importante, elle est de 39,9 % chez les hommes et de 33,4 % chez les femmes, seulement 35,5 % se savaient hypertendus, le pourcentage des sujets traités parmi les hypertendus est élevé (34,4%), le contrôle thérapeutique de la PA reste très faible, seulement 25% ont une PA 140/90mmHg. La comparaison de notre population avec celles décrites dans la littérature en ce qui concerne les facteurs de risque met en évidence les résultats suivants. La prévalence du tabagisme est faible, il n'y a pas de différence significative dans la prévalence des dyslipidémies, la proportion d'obèses est significativement plus élevée qu'en métropole ou en Afrique mais reste inférieure à celle des Etats-Unis, la prévalence de l'hyperglycémie/diabète est forte, deux fois supérieure à celle de métropole et il existe une forte prévalence de l'hypertension significativement supérieure à celle des populations d'Afrique, des Caraïbes anglophones et des Etats-Unis. Notre recherche permet néanmoins de montrer la nécessité de dépister les facteurs de risque cardiovasculaire en mettant en évidence ; d'une part, la méconnaissance de ces désordres chez les sujets, en effet ont été dépisté et orienté vers leur médecin traitant ou un spécialiste, 950 sujets pour la prise en charge d'une dyslipidémie, 195 personnes pour un contrôle d''hyperglycémie et 1726 sujets pour le contrôle d'une PA140/90 mmHg ; d'autre part la fréquente association des facteurs de risque : 66,2 % des hommes et 63,7 % des femmes ont plus d'un facteur de risque.Et enfin, la nécessité d'un meilleur contrôle thérapeutique. Ainsi nous espérons que ces résultats contribueront à la généralisation des campagnes de dépistage et de prévention des maladies cardiovasculaires dans ce département.

Having safe drinking water is important to all Americans. The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.

In AD 42, the Emperor Claudius initiated work on the construction of a new artificial harbour a short distance to the north of the mouth of the Tiber. The harbour facilities were enlarged at the instigation of the Emperor Trajan at the beginning of the second century AD, and Portus remained the principal port for the City of Rome into the Byzantine period. The surviving archaeological remains and comments by ancient sources make it clear that Portus lay at the heart of Rome's maritime façade. As well as being a key Mediterranean centre for passengers and for the loading, unloading, transshipment and storage of products from across the Empire, it was also designed to make an ideological statement about the supremacy of Rome in the world. Portus is, thus, of key importance to understanding Rome and her relationship to the Empire. The project that forms the subject of this book was designed to use non-destructive techniques of topographic and geophysical survey in combination with systematic surface collection to provide a new understanding of the plan of Portus. The work was undertaken between 1997 and 2002 as a collaboration between the Soprintendenza per i Beni Archeologici di Ostia, the British School at Rome, and the Universities of Southampton, Durham and Cambridge. This volume presents the full results of the survey and uses them as the basis for a re-evaluation of the whole port complex. The geophysical survey results are interpreted in the context of earlier work at the site in order to offer new perspectives on the character and development of the site.
