The FAA uses the Air Transport. Oversight System (ATOS) to oversee 7 legacyÓ airlines & 9 other airlines. This report refers to airlines that are not in ATOS as non-legacy airlines. Two other processes are used to oversee 99 non-legacy passenger airlines (NLPA), which represent a fast-growing segment of the commercial aviation passenger industry & carried 200 million passengers. in 2004. These establish a set of inspection activities for NLPA, & use principles of system safety to identify additional risk-based inspections for those airlines. Human error is implicated in nearly all aviation accidents, yet most investigation and prevention programs are not designed around any theoretical framework of human error. Appropriate for all levels of expertise, the book provides the knowledge and tools required to conduct a human error analysis of accidents, regardless of operational setting (i.e. military, commercial, or general aviation). The book contains a complete description of the Human Factors Analysis and Classification System (HFACS), which incorporates James Reason's model of latent and active failures as a foundation.
