Extracorporeal photopheresis (ECP) is the first FDA approved cellular photoimmunotherapy for cancer, namely cutaneous T-cell lymphoma, and has demonstrated efficacy in various diseases that have a suspected involvement of pathogenic T-cells including prevention and treatment of acute and chronic graft-versus-host disease, organ transplant rejection, selected autoimmune diseases and Sezary syndrome. In ECP, patient’s blood mononuclear cells are collected, exposed to ultraviolet light in the presence of extracorporeally administered liquid 8-methoxypsoralen and reinfused. Besides its considerable efficacy ECP has an exceptional safety profile, does not cause general immunosuppression and thus, does not increase risk of infectious complications and relapse of malignant disease. Exciting preclinical data and clinical observations provide insight into the mechanisms of action of ECP and support its immunomodulatory role. This book will provide an overview on the historical development of ECP, technical aspects for its use in various patient populations including children and adults, novel research findings both in preclinical models and in various patient cohorts and will stimulate further research in organ, tissue and cell transplantation and autoimmune disorders.

Steroids are the first line but more than 40% of these patients will not response.OBJECTIVE: Evaluate efficacy and safety of ECP for refractory GVHD.DESIGN: retrospective study of 84 patients treated with ECP between january-2010-april-2018 in our centre.PATIENTS: 84 patients with steroid-refractory acute/chronic GVHD after alloSCT. Patients who didnu00b4t complete 8 procedures were not evaluable.INTERVENTIONS: an integrated system of continue flow, processing a standard volumen. Patients with aGVHD received 2 consecutive sessions/week during 12 weeks, then 2 sessions/fortnight until stop steroids. Protocol for cGHVD was 2 sessions/fortnight during 12 weeks and 2 sessions/month during 3 months until stop steroids.MAIN OUTCOMES MEASURES: ORR, OS and security.RESULTS: -aGVHD(n=35): median age of 48 (11-71) and 65.7% men. Organ involved was skin (91.4%), gut (71.4%) and liver (40%). 15 patients present grade II, 3 grade III and 17 grade IV. ORR was 78.1% (CR:53.1%). All of patients with grade II-III responded, but only 10 of 17 patients with grade IV GVHD. Best organ response was skin (ORR:87.1%). Grade IV had and adverse outcome (median OS not reached VS 3 months, p=0.016) and CR after ECP was associated with favourable outcome (median OS not reached VS 6 months, p=0.061). With a median follow-up after ECP of 5 months (0-51), 51,4% live.-cGVHD(n=49):, median age of 55 (9-69) and 54% men. 24 had severe cGVHD. Principal organs involved were skin and mucosas. ORR was 69%(CR:24.4%). Responses of moderate was superior (80%). Best organ responses were oral (ORR:84%) and skin (ORR:80%). After a median follow-up of 29 months (3-96), OS after ECP at 2 years was 78.3%.- Security: 8 CMV reactivation and 2 fungical infections ocurring during ECP. Steroids could stop in majority of the patients. 6 GVHD reactivacion were observed during ECP, the majority respond with intensification.CONCLUSIONS: ECP is an effective and safe salvage therapy for GVHD, especially in grade II-III aGVHD and moderate cGVHD. Best organ responses are skin in aGVHD and skin and mucosa in cGVHD.
