Probablemente exista un acuerdo casi unánime respecto a la necesidad de fundamentar científicamente las decisiones que se adoptan en los distintos procedimientos aplicados en el marco de la atención sanitaria. Algunas personas lo darán por hecho, otras serán algo más escépticas. Estas decisiones no solo hay que tomarlas sobre cómo afrontar los problemas, sino también, y previamente, sobre cómo entender los problemas, porque la manera de explicar los problemas de salud y sus causas condicionará la respuesta que les demos. Las distintas explicaciones a esos determinantes y las desigualdades sociales en salud nos proporcionarán una base para dar algunas respuestas a la pregunta del título, pero hay otras intermedias: ¿Son las respuestas que damos a los problemas de salud las más adecuadas? ¿Van a la raíz de los mismos? ¿Mejoran a corto, medio y largo plazo no solo el problema puntual, sino la salud global de una persona, de su familia o del conjunto de la población? Los factores del entorno social y físico, como la cohesión social, la contaminación, el ruido, los espacios verdes, la facilidad para caminar o ir en bici son determinantes en los estilos de vida que, a su vez, influyen de manera importante en la salud. Modificar esos factores ambientales contribuirá a que la gente pueda elegir un estilo de vida saludable, utilizando además las estrategias educativas pertinentes. Revisaremos los orígenes, logros y retos de lo que supuso un intento formal de aplicación de la evidencia científica a los problemas de salud, lo que se ha llamado atención sanitaria basada en la evidencia, que integra la experiencia profesional, las preferencias de las personas atendidas y la evidencia procedente de la mejor investigación clínica.
