Contexte : le carnet de santé des enfants existe depuis 1945. Il est l'unique support de suivi inter disciplinaire des enfants. La dernière version date de 2006. A l'occasion du projet de nouveau carnet de santé en 2017, il semble nécessaire de l'évaluer. Objectif : évaluation du remplissage du carnet de santé des enfants de 0 à 6 ans, de sa conformité par rapport aux recommandations et de son lien avec l'état de santé de l'enfant. Méthode : étude quantitative rétrospective multicentrique par analyse du contenu des carnets de santé des enfants de 0 à 6 ans en Loire Atlantique à l'aide d'un questionnaire, entre mai et décembre 2016. Résultats : le remplissage global des carnet de santé est majoritairement totalement conforme aux recommandations(urgences 20 (40%), pédiatrie 32 (65,3%), médecine générale 21 (38,9%)) sauf pour la PMI avec une majorité de partiellement conforme 18 (45%).Cependant c'est aussi la PMI qui paradoxalement compte le plus petit taux de non conformes : 2 (5%). Il y a une différence significative (p=0,005) entre l'âge des enfants dont le carnet est bien rempli (16,6 mois ± 14,2) et ceux dont le carnet est mal rempli (20 ,9 mois ± 18,9). La santé globale est optimale dans 186 cas (96,4%). Aucun lien ne semble établi entre le remplissage du carnet de l'enfant et son état de santé sauf en PMI, que ce soit pour dire que c'est en grande partie lié (12 (30,8%)) ou pour dire qu'il y a un peu de lien (11 (28,2%)) ou pour dire qu'il n'y a pas du tout de lien (2 (5,1%)). Conclusion : il apparaît de nombreuses lacunes dans l'enregistrement de données importantes pour le suivi des enfants, surtout chez les plus âgés. A l'aube du carnet de santé 2017, des informations à l'attention des médecins et des parents devraient conduire à une meilleure utilisation du carnet de santé.

INTRODUCTION : La surveillance pondérale des enfants est un élément systématique de l’examen clinique. La qualité du suivi pondéral n’a cependant pas été évaluée dans cette population pédiatrique suivie en médecine générale. OBJECTIF : Evaluer les pratiques concernant le suivi pondéarl des enfants en médecine de ville : la méthode de pesée, le suivi des courbes pondérales et la fréquence des pesées et déterminer l’influence sur ce suivi de facteurs comme le milieu socioprofessionnel de la famille, les antécédents de l’enfant ou la spécialité du médecin suivant l’enfant. METHODE : Etude transversale descriptive concernant les enfant âgés de 0 à 6 ans consultatnt en service d’urgence pédiatriques sur un mois, par entretien avec les parents puis étude du carnet de santé. RESULTATS : 107 enfants ont été inclus. Le suivi pondéral des enfants est inégal dans la population étudiée. Les courbes pondérales n’étaient faites que sur 65.4% des carnets analytsés et étaient incomplètres dans 51.4% des cas. 34.6% des enfants n’étaient pas entièrement déshabillés lors de la pesée. Il existe une inégalité sociale de santé dans la qualité du suivi : même si 60.5% des enfants aivaient eu un suivi conforme aux recommandations de la HAS, cela ne concernait que 46.7% des enfants d’employés, et 42.9% des enfants de commerçants et d’artisans, alors que plus de 80% des enfants de cadres, professions intellectuelles supérieures et professions intermédiaires étaient suivis correctement. CONCLUSION : L’étude pose la question des inégalités sociales de santé dans le suivi pondéral des enfants et de l’évolutoin du carnet de santé, vecteur de ce suivi
