Au CHU de Nantes, s'est ouvert, en janvier 2013, un service d'hôpital de jour dédié spécifiquement aux adolescents avec maladies chroniques. Ils y bénéficient d'une prise en charge pluridisciplinaire, novatrice et originale. L'objectif principal de cette étude est d'évaluer l'impact de 3 mois de prise en charge en HDJ Adolescent sur la qualité de vie des adolescents suivis. Il s'agit d'une étude préliminaire à une étude prospective, menée actuellement dans le service. Ont été inclus dans l'étude les 13 adolescents entrés à l'HDJ Adolescent de juillet à novembre 2013. Le questionnaire de qualité de vie VSP-A leur à été remis, ainsi qu'à leurs parents, lors de l'entrée (M0) puis à 3 mois (M3) de prise en charge. Les patients ont été classés en 3 groupes selon la pathologie: maladies chroniques, trouble du comportement alimentaire et troubles somatomorphes. Nous avons mis en évidence une amélioration importante du score global de qualité de vie entre M0 et M3, selon les adolescents, et leurs parents. L'amélioration la plus importante concernait les groupes maladies chroniques et trouble du comportement alimentaire. Hormis concernant leur relation parents-adolescents il existait peu de discordance entre les scores des adolescents et ceux de leurs parents. Bien que la cohorte soit de petite taille, sa composition est le reflet des effectifs de l'HDJ adolescent. La prise en charge pluridisciplinaire, conjointe (pédiatrique et pédopsychiatrique), qui met en avant les groupes de pairs et les médiations thérapeutiques, sans négliger les problématiques somatiques de ces patients lourds, apparait comme une réponse thérapeutique adaptée, qui offre le meilleur soin possible à ces adolescents.

Les passages aux urgences et hospitalisations pour tentative de suicide (TS) sont en augmentation constante ces dernières années en France. Ils touchent en particulier les jeunes filles entre 15 et 19 ans. Le recours aux professionnels de santé dans le mois précédent l'hospitalisation pour TS et les prises en charges psychologiques préexistantes sont peu étudiées dans la littérature sur la population adolescente. Notre étude prospective s'est intéressée aux parcours de soins de 62 mineurs âgés de 11 à 17 ans et admis au CHU de Nantes pour TS ou idéations suicidaires. 82 % de nos patients ont rencontré un professionnel de santé ou du système scolaire le mois précédent leur admission au CHU et 61% leur médecin généraliste. Deux tiers des jeunes ont confié leur souffrance psychique lors de cette rencontre. 44 % de ces mineurs avaient déjà un suivi psychologique en cours avant l'épisode actuel. En dehors de la crise suicidaire, les adolescents consultent assez régulièrement leur médecin mais lui confient difficilement leur mal-être. Ils en ont une vision assez « somaticienne ». Le médecin traitant doit donc rechercher activement une éventuelle souffrance lors de toute consultation avec un jeune patient. Après une TS, il a un rôle central dans la coordination des soins, par sa connaissance de l'environnement de l'enfant et de son entourage familial. Il assure le relais entre l'hôpital et le démarrage d'un suivi ambulatoire, et par la suite, il doit rester vigilant quant au risque de récidive. Pour l'aider dans ces missions de repérage et de prévention, le développement d'outils pédagogiques simples d'utilisation et des formations professionnelles adaptées seraient à mettre en place.

This book is concerned with children's economic and social rights (sometimes referred to simply as children's social rights). Despite increased academic interest in both children's rights and socio-economic rights over the last two decades, children's social and economic rights remain a comparatively neglected area. This is particularly true with regard to the role of the courts in the enforcement of such social rights. Aoife Nolan's book remedies this omission, focussing on the circumstances in which the courts can and should give effect to the social and economic rights of children.
