Les infections liées à la voie veineuse centrale (ILVVC) sont la complication la plus fréquente de la nutrition parentérale à domicile (NPAD). Les taux d'ILVVC sont variables dans la littérature en fonction de la définition de l'ILVVC, des caractéristiques des patients et de l'expérience de l'équipe manipulant la VVC. Cette étude rétrospective monocentrique évalue les enfants avec de la NPAD de janvier 2014 à décembre 2019 dans un centre expert de NPAD pédiatrique. Des données cliniques, nutritionnelles et biologiques ont été collectées. Les patients ont été répartis en deux groupes : les patients avec des infections récurrentes (RCT) s'ils avaient eu au moins deux ILVVC en six mois, et le groupe contrôle (CTL). Des analyses de régression univariées et multivariées ont été réalisées sur les patients non appariés, puis appariés sur leur durée de VVC afin d'éliminer un possible biais. Dans cette étude de 75 patients en NPAD, le taux d'ILVVC était de 1,74/1000 jours de VVC. Avant appariement, un risque d'infections récurrentes était significativement associé à la durée de VVC (OR=1.04, IC 95% [1.01-1.06], p=0.004) et à l'hébergement au long terme en soins de suite et de réadaptation (OR=6, IC 95% [1.5-26.6], p=0.012). Après appariement, seul le deuxième critère était significativement associé à un risque d'infections récurrentes (OR=6.27, IC 95% [1.21-32.5], p=0.03). Il s'agit d'un établissement d'hébergement d'enfants ne pouvant être traités par nutrition parentérale dans leur domicile familial pour des raisons sociales, mais sans que leur pathologie soit plus sévère que les autres enfants en NPAD. Notre principale hypothèse pour expliquer ce surrisque est que les parents et aidants reçoivent une formation approfondie par une équipe multidisciplinaire et peuvent concentrer leur attention sur leur enfant, tandis que les infirmières d'un établissement de soins de réadaptation doivent s'occuper de plusieurs patients. Notre étude suggère que des mesures préventives devraient être mises en œuvre, en particulier dans les établissements de soins de suite et de réadaptation afin de réduire la proportion d'enfants avec des infections récurrentes. Une étude multicentrique est nécessaire pour confirmer nos résultats dans une cohorte plus importante.
