Dans le cadre de la mise en place de sa nouvelle unité de reconstitution centralisée des cytostatiques (URCC), la pharmacie Logipôle Trousseau (Tours, France) a conçu un programme spécifique de contrôle qualité physico-chimique des préparations pour chimiothérapie. Le 5-fluorouracile (5FU), agent cytotoxique le plus prescrit au sein de l'établissement, est à la base de ce programme de contrôle validé parallèlement à deux études menées au sein de l'URCC et du laboratoire d'analyses et de contrôle qualité de la pharmacie Logipôle Trousseau : 1) une étude de la stabilité du 5FU dans le sérum glucosé à 5% (G5), réalisée à l'issue de la validation locale du dosage du 5FU. En l'absence de données exploitables, la stabilité du 5FU a été testée en fonction du mode de conditionnement (poche, infuseur ou cassette pour pompe), de la température de stockage et de la concentration en 5FU sur une durée totale de 14 jours et ; 2) la validation des protocoles de fabrication des cytotoxiques réalisée à partir de 40 préparations (poche : n=20 ; infuseurs : n=10 ; cassette : n=10) contenant du 5FU et la détermination des normes d'acceptabilité des préparations. Les résultats obtenus ont permis de conclure à la stabilité du 5FU dans le G5, l'ensemble des paramètres de contrôle mesurés (limpidité ; pH ; concentration en 5FU) n'ayant pas été significativement perturbés au cours de l'étude. D'autre part, les taux d'erreurs moyens des concentrations de 5FU (+/- écart type) mesurés dans les préparations ont été de 1,35 +/- 2,59%, 0,52 +/- 1,46% et 0,73 +/- 2,28% respectivemnet dans les poches, infuseurs et cassettes pour pompes. Ce programme de contrôle original, fondé sur l'examaen visuel de l'ensemble des préparations et sur le dosage de toutes les préparations contenant du 5FU permet, en pratique, de vérifier la conformité à la prescription d'environ 60% dese préparations et de valider quotidiennement le processus de reconstitution des cytostatiques. Les résultats des dosages des prescriptions réalisées dns le cadre de la validation des modes opératoires de l'URCC ont permis de fixer un taux d'erreur maximul à la prescription tolérée à 5%. Le dépassement de ce seuil de tolérance et la gestion du risque iatrogène associé soulèvent le problème de l'exploitation des résultats du contrôle et doivent faire l'objet d'une concertation entre le clinicien et le pharmacien.

Evaluates the carcinogenic risks to humans posed by the use of four antiretroviral agents four DNA topoisomerase II inhibitors used in the treatment of cancer and an additional three pharmaceutical agents (hydroxyures phenolphthalein and vitamin K substances). The volume marks the first IARC evaluation of nucleoside analogs that act as antiviral agents. The evaluation responds in part to recent findings that zidovudine (AZT) an effective antiretroviral agent now being given to pregnant HIV-infected women to prevent maternal-to-fetal transmission of the virus is a transplacental carcinogen in mice. The opening monograph evaluates the carcinogenicity to humans of the antiretroviral nucleoside analogs zidovudine (AZT) zalcitabine (ddC) and didanosine (ddI) and the antiherpesvirus drug aciclovir. Of these aciclovir and didanosine could not be classified on the basis of available data. For zidovudine transplacental administration to mice resulted in an increased incidence and multiplicity of lung and liver tumours and in an increased incidence of female reproductive tract tumours in one study but not in another involving treatment at a lower dose. Despite observation of toxic effects in some studies of humans human carcinogenicity data were judged to provide inadequate evidence of carcinogenicity in humans. Zidovudine was classified as possibly carcinogenic to humans. Similar weaknesses in human carcinogenicity data for zalcitabine which consistently induces thymic lymphomas in mice resulted in its classification as possibly carcinogenic to humans. The second monograph evaluates four DNA topoisomerase II inhibitors: etoposide teniposide mitoxantrone and amsacrine. Of these etoposide - one of the most widely used and effective cytotoxic drugs in combination therapy - was classified as probably carcinogenic to humans and etoposide in combination with cisplatin and bleomycin was judged to be carcinogenic to humans. Teniposide was classified as probably carcinogenic to humans and mitoxantrone and amsacrine were classified as possibly carcinogenic to humans. Of the three pharmaceutical agents evaluated in the final monograph hydroxyurea which is widely used in cancer treatment and increasingly in combination with didanosine in HIV infection could not be classified. Phenolphthalein a widely used laxative now being withdrawn from the market in many countries because of toxicological concerns was classified as possibly carcinogenic. Vitamin K substances could not be classified on the basis of available evidence.

Twenty years ago, the enzyme superoxide dismutase which uses the superoxide radical anion as its specific substrate was reported. With this discovery was born a new scientific field, in which oxygen, necessary for aerobi c 1 ife on thi s planet, had to be cons i dered also in terms of its toxicity and stresses. This stimulated the search for knowledge of active oxygen species in biology and medicine. Superoxide and other reactive oxygen species are now implicated in many disease processes. Major advances have been achieved during these past years with respect to free radical generation and mechanisms of free radical action in causing tissue injury. In parallel, the possibil ity of influencing free radical related disease processes by antioxidant treatment was studied in various in vitro and in vivo systems. This was the unique theme of a conference organized in Paris by the Society for Free Radical Research (December 9-10, 1988) which brought together experts from basic sciences and clinicians in order to evaluate the current status of antioxidant therapy. The conference emphasized fundamental processes in antioxidant action. Among the major topics were superoxide dismutase (SOD) and low molecular weight substances with such activity, called SOD mimics. Other antioxidant enzymes were also considered.
