Les médecins généralistes sont amenés à prendre en charge des diabétiques originaires d’Afrique de l’Ouest. Leur mode de vie, leur alimentation, les idées reçues et conceptions préexistantes à la maladie nous sont inconnus alors qu’ils sont nécessaires à une éducation thérapeutique efficace. L’analyse de six focus groups de participants originaires d’Afrique de l’Ouest m’a permis de mettre en évidence les principaux axes de réflexion concernant les représentations liées au diabète. Les représentations étiologiques sont centrées en priorité sur une origine exogène du diabète faisant référence à un mode de vie occidental subi. L’abandon des habitudes de vie, de l’alimentation et des thérapeutiques traditionnelles est incriminé dans la genèse de la maladie. Pourtant l’alimentation africaine est perçue comme incompatible avec un régime diabétique. La perspective d’un abandon des habitudes alimentaires pour un régime diabétique est perçue comme une déculturation chez des patients qui ont déjà dû subir des modifications profondes de leur mode de vie lors de leur immigration. La recherche d’une guérison complète face au diabète peut aussi représenter un obstacle à la prise en charge : les patients abandonnent facilement un traitement qu’ils jugent inefficace et trop contraignant ou bien préfèrent se tourner vers des tradipraticiens. Ces représentations diffèrent d’un patient à l’autre car elles sont influencées par le vécu, le milieu socioprofessionnel et les croyances religieuses. Leur recherche et leur intégration à la prise en charge du patient diabétique sont essentielles.

Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

While working in Africa, anthropologist Julien Bonhomme encountered an astonishing phenomenon: people being accused of stealing or shrinking the genitals of strangers on the simple occasion of a handshake on the street. As he soon discovered, these accusations can have dramatic outcomes: the "sex thieves" are often targeted by large crowds and publicly lynched. Moreover, such rumors are an extremely widespread practice, having affected almost half of the African continent since the 1970s. In this book, Bonhomme examines the story of the "penis snatcher," asking larger questions about how to account for such a phenomenon--unique in its spatial and temporal scale--without falling prey to the cliché of Africa as an exotic other. Bonhomme argues that the public belief in sex thieves cannot be considered a superstition or form of mass hysteria. Rather, he brings to light multiple factors that explain the rumor's success and shows how the cultural dynamic can operate on a vast scale. Analyzing the rumor on both transnational and local levels, he demonstrates how it arises from the ambiguities and dangers of anonymity, and thus that it reveals an occult flipside to everyday social interaction. Altogether, this book provides both richly ethnographic and theoretical understandings of urban sociality and the dynamics of human communication in contemporary Africa and beyond.
