La maîtrise de la contamination microbiologique ou biocontamination est un enjeu majeur dans le cadre des préparations stériles pour les pharmacies hospitalières. La connaissance de l'environnement microbiologique des milieux de préparation est essentielle pour garantir leur stérilité. Pour cela, la surveillance de l'environnement des zones à atmosphère contrôlée (ZAC) est une obligation selon les Bonnes Pratiques de Préparations (BPP) en pharmacie hospitalière. Les données issues des modalités de surveillance sont enregistrées dans des documents et ne sont pas suffisamment utilisées. Face à ce constat, il a été proposé à partir des données des prélèvements microbiologiques du laboratoire de préparation de poches de nutrition parentérale une interprétation des résultats dont l'objectif était d'évaluer l'incidence des pratiques de préparation et le comportement sur la biocontamination en ZAC. En suivant un plan d'échantillonnage, des géloses de sédimentation ont été placées dans la ZAC pendant les périodes d'activité de 4h. Des prélèvements sur les gants des opérateurs en fin d'activité a également été effectué. Les géloses ont ensuite été incubées en zone tempérée entre 15 et 20°C pendant 4 jours +/- 1 jour puis à l'étuve entre 30 et 35°C pendant 3 jours +/- 1 jour. Les prélèvements positifs ont ensuite été envoyé au laboratoire de microbiologie pour identification. A partir du nombre de prélèvements positifs détectés dans l'environnement et sur les gants des opérateurs, du nombre de non-conformité, et l'identification des microorganismes, une évaluation de la surveillance de la biocontamination en ZAC au laboratoire de préparation de poches de nutrition parentérale a été réalisée. Au total 12 196 prélèvements microbiologiques ont été effectués au laboratoire de nutrition parentérale. Parmi eux 1752 échantillons positifs ont été analysés. La mise en évidence et l'identification des microorganismes présents a permis de définir l'environnement microbiologique et visualiser l'impact des pratiques professionnelles dans un laboratoire de préparation stérile. Le suivi des résultats conformément au BPP a permis de démontrer que l'hygiène des locaux, équipement ainsi que des opérateurs avaient une influence sur l'environnement microbiologique. Les modalités de nettoyage et désinfection ainsi que les tenues vestimentaires différentes entre les classes ont montré leur influence sur l'environnement tout comme le procédé de préparation stérile.
