Introduction : Les plans nationaux Cancer successifs placent le médecin généraliste (MG) comme pivot dans la prise en charge des patients atteints de cancer. Lors de la phase active du traitement du cancer, des difficultés majeures apparaissent dans la qualité de l'information transmise et son moyen de communication au MG. Objectif : Réaliser un état des lieux portant sur les outils de coordination expérimentés pour améliorer les échanges entre le système hospitalier et le MG dans la prise en charge du cancer durant la phase active de traitement. Matériel et méthode : Revue systématique de la littérature en interrogeant les bases de données MEDLINE via PubMed, The Cochrane Library, Web Of Science entre 1998 et 2018. Résultats : Onze études ont été incluses. Elles soulignent une majoration de la qualité de prise en charge des patients à l'instauration de fiches d'informations ou de vidéos vignettes. Elles démontrent l'importance de l'utilisation de courriers standardisés entre les professionnels de santé. Des initiatives de coordination de soins y sont retrouvées notamment au Canada. Le rôle du " médecin leader " y est abordé et ses premières évaluations sont positives. Cependant, le manque de lien entre le MG et l'infirmier diplômé d'état de coordination en cancérologie y est dénoncé. Discussion : La majoration de l'information vers les MG, entraine une élévation de la satisfaction globale des patients et des MG sur la prise en charge, mais aussi une augmentation du nombre de consultations vers le MG et une diminution du nombre d'hospitalisations consécutives à un effet indésirable des traitements anti néoplasique. Les outils de communication nombreux sont essentiels à la transmission de l'information. Cependant il ne faut pas oublier l'importance de la communication directe et orale entre l'ensemble des professionnels de santé. Conclusion : L'amélioration de l'information transmise et des échanges via des outils de communication multiples vers le MG permettent une meilleure qualité globale de prise en charge.

INTRODUCTION : En 2017 on dénombre près de 400 000 nouveaux cas de cancers en France. Avec un taux de mortalité de 124 pour 100 000 hommes, et 72,9 pour 100 000 femmes, le cancer est, en 2015, la première cause de mortalité en France. On note plus de 25% de troubles de l'adaptation avec humeur dépressive chez les personnes atteintes de néoplasie. Le postulat avancé est qu'une intervention du médecin généraliste sous forme de consultation au cabinet tend à diminuer les symptômes et plaintes du patient à la phase active de son traitement. L'objectif principal est de créer un outil permettant d'évaluer l'impact de l'implication du médecin généraliste sur l'expérience du patient dont l'état psychologique requiert une prise en charge. MÉTHODE : L'évaluation se fera à l'aide de l'échelle de la qualité de vie ayant déjà fait ses preuves : la SF-12, ainsi que d'un recueil de données permettant une évaluation qualitative de la souffrance psychologique. Pour valider ce recueil de données, nous avons eu recours à la méthode de consensus Delphi. 23 experts ont été recrutés après contact téléphonique : MSU, médecins généralistes installés en Poitou-Charentes entre 2007 et 2016, ayant donné leur consentement oral. RESULTATS : Le consensus a été obtenu au terme de 2 rondes de Delphi. Il a permis la constitution d'un recueil de données comprenant 10 items validés par le panel d'experts. Ce recueil sera proposé lors des consultations de suivi programmées au sortir d'hospitalisation pour les patients nouvellement traités pour leur cancer. PERSPECTIVE: Au terme de cette étude, les médecins généralistes disposeront d'un outil d'évaluation ambulatoire comprenant une dimension quantitative avec le questionnaire SF-12, et une dimension qualitative avec le recueil de données issu de la méthode Delphi. Cet outil sera proposé lors de consultations de suivi dédiées chez le médecin généraliste et permettra d'évaluer l'impact de l'intervention du médecin traitant sur la qualité de vie et la satisfaction du patient à la phase active de son traitement.

Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being.
