INTRODUCTION ET OBJECTIFS : Mise en place d'une formation à l'annonce d'une mauvaise nouvelle pour les internes en pédiatrie par simulation d'entretien : validation d'une grille d'évaluation et évaluation de la formation par courbes de compétences, sentiment d'efficacité personnelle et satisfaction des apprenants. MÉTHODE : Une grille de 25 items a été conçue à partir des recommandations HAS et internationales puis améliorée dans son contenu par des experts selon la méthode Delphi modifiée. La validité de construit a été testée en comparant les scores obtenus par 4 groupes de niveau sur un exercice de simulation d'entretien avec un patient standardisé : des étudiants en 4ème année de médecine, des internes en 1ère année d'internat de pédiatrie, des internes de dernière année d'internat de pédiatrie et des séniors experts. La validité d'apparence et la fidélité inter-juges ont été recherchées en comparant les notes et avis de 4 observateurs indépendants. La formation « annonce d'une mauvaise nouvelle en pédiatrie » a été conçue à l'aide d'un expert en annonce, oncopédiatre et d'un expert en pédagogie par la simulation. Elle comporte une formation théorique vidéo avec saynètes puis 2 sessions pratiques formatives avec jeux de rôles. Un groupe « intervention » d'internes en début d'internat a bénéficié de la formation, puis a été évalué avec la grille préalablement validée. Les notes du groupe « intervention » ont été comparées avant et après formation et à celles obtenues par un groupe « référent », composé de 11 étudiants en fin d'internat de pédiatrie. Leur sentiment d'efficacité personnelle a été mesuré avant et après formation ainsi que leur satisfaction à l'issue de la formation. RÉSULTATS : 30 participants ont fait l'exercice de simulation pour validation de la grille. 9 experts ont validé l'intégralité de la grille. Il y avait une différence significative entre les notes globales des 4 groupes (p = 0.009) avec une note moyenne la plus haute pour les experts, puis décroissante à mesure que l'expérience des étudiants diminuait. La corrélation alpha entre les évaluateurs était > 0.7 pour toutes les combinaisons d'évaluateurs et le test de Spearman montrait une corrélation significative pour tous les observateurs. La note moyenne de compétences du groupe « intervention », étudiants de début de cursus avant formation était de 26,14/50, celle des étudiants de fin de cursus (groupe référent) était de 31,05/50. The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats.
