Introduction : la fièvre est un motif de consultation fréquent en pédiatrie. Le paracétamol reste l'antipyrétique de premier choix, mais l'utilisation de l'ibuprofène augmente. Objectif : évaluer le profil de consommation des AINS chez l'enfant de 3 mois à 16 ans se présentant pour fièvre aux Urgences Pédiatriques. Matériel et méthodes - Il s'agit d'une étude observationnelle réalisée aux urgences pédiatriques du Centre Hospitalier Universitaire (CHU) de la Timone à Marseille, sur une période de 6 mois. Nous avons inclus les enfants de 3 mois à 16 ans se présentant aux urgences pour une fièvre > 38°C. Un questionnaire était rempli aux urgences regroupant des données démographiques, l'état clinique à l'arrivée, des données relatives à une prise d'ibuprofène ou d'autres médicaments, la prise en charge diagnostique et thérapeutique de l'enfant. Résultats : sur les 162 enfants inclus dans notre étude, 31 (19,1%) étaient exposés aux AINS au cours de l'épisode fébrile. L'ibuprofène était l'AINS le plus administré (n=24 soit 77,4%). Vingt enfants (64 %) ayant été exposé aux AINS avaient reçu un autre antipyrétique en association. Parmi les 162 patients inclus, 5 présentaient une complication infectieuse au décours de l'épisode aigu. Conclusion : les AINS sont souvent administrés hors prescription, en bithérapie (avec le paracétamol) et sans connaissance des effets secondaires de la part des parents. La majorité des enfants de notre étude n'avaient aucun critère de fièvre mal tolérée. Une monothérapie par paracétamol aurait été suffisante. Il est à ce jour difficile de prouver, le lien ou l'absence de lien, entre l'exposition des AINS au cours d'un épisode infectieux fébrile et la survenue de complications infectieuses.

Introduction : L'automédication des patients par anti -inflammatoires non stéroïdiens (A I NS) nécessite leur éducation, compte-tenu du risque du risque d'effets indésirables (El). L'objectif de cette étude évaluait l'impact dans le temps d'un outil d'information écrite concernant les El des AINS sur les connaissances de patients consultant en médecine généra le et déclarant s'automédiquer. Méthode : Il s'agissait d 'une étude prospective, de type avant/après, menée de novembre 2016 à janvier 201 7, dans la maison de santé rurale bourguignonne de Luzy. Les patients consultant leur médecin généraliste, ont rempli un questionna ire écrit, évaluant leurs connaissances par l'intermédiaire d'un score sur onze. Ils se voyaient remettre ensuite l'outil d'information écrite sur les El des AI NS, que nous av ions élaboré à partir d'une fiche d'information britannique déjà validée. Le critère principal d'évaluation était l'évolution des connes naissances, mesurées via ce même questionnaire par entretien téléphonique une semaine et un mois après remise de l'outil. Résultats: Les scores de connaissances des 223 patients inclus étaient significativement supérieurs une semaine (+2,82/11) et un mois (+3,1 4/11) après remise de l'information écrite (p=0.001). Les patients semblaient s'informer essentiellement par leur médecin, pharmacien ou via les notices des médicaments. Conclusion : L'outil d'information écrite proposé améliorait les connaissances des El des AJ NS au sein d'une population consultant en soins premiers et déclarant s'automédiquer. L'usage en pratique courante pourrait aider les praticiens, mais nécessiterait un travail de validation.
