There is great enthusiasm over the use of emerging interactive health information technologies-often referred to as eHealth-and the potential these technologies have to improve the quality, capacity, and efficiency of the health care system. However, many doctors, advocacy groups, policy makers and consumers are concerned that electronic health systems might help individuals and communities with greater resources while leaving behind those with limited access to technology. In order to address this problem, the Institute of Medicine's Roundtable on Health Literacy held a workshop to explore the current status of communication technology, the challenges for its use in populations with low health literacy, and the strategies for increasing the benefit of these technologies for populations with low health literacy. The summary of the workshop, "Health Literacy, eHealth, and Communication: Putting the Consumer First," includes participants' comments on these issues.

This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

E-health technologies are explicitly designed for facilitating challenges in the health care setting. E-health encompasses a variety of sophisticated technologies from telemedicine, electronic health records (EHR), mobile health, and a wide range of health services. The main goal of implementing e-Health is improving the quality of patient care by minimizing medical errors, increase the quality of data, sharing information between healthcare providers and patients, and collecting health information for educational and research purposes (Parker and Ruth, 2000). Unfortunately, vulnerable populations are at the most considerable risk for lack of literacy, misunderstanding their medical diagnoses, medications, and instructions on how to take care of their chronic health issues. E-health is an increasingly popular way for people to obtain their personal health information, but several systemic barriers prevent people from accessing electronic health information. This study will analyze factors associated with equitable access to health care through the use of e-Health. The findings of this study will be useful to healthcare organizations that are in the process of adopting a renewed approach to e-Health training and teaching and its effect on patient literacy.
