La question de la prise en charge de la douleur fait désormais partie du débat public. Par quels cheminements la douleur, tout à la fois problème fondateur de la médecine et zone d'ombre de son développement le plus scientifique, arrive-t-elle aujourd'hui sur le devant de la scène ? La douleur et la médecine sont-elles en train de nouer de nouveaux rapports? Comment organiser un travail médical nouveau sur la douleur elle-même, alors qu'en dernière analyse le malade est seul à pouvoir juger de " sa " douleur ? Ce livre tente d'éclairer la situation présente de la douleur comme objet de la pratique médicale et sa genèse. Il s'appuie sur un travail de terrain de plusieurs années : auprès des fondateurs des premières " consultations de la douleur " françaises de l'Assistance publique et en tout premier lieu du Dr John Bonica, anesthésiste américain, entrepreneur charismatique de la médecine de la douleur. L'auteur a ensuite sélectionné deux centres de la douleur emblématiques des approches contrastées qui partagent ce domaine et y a entrepris une observation de plus de huit mois de consultations médicales ainsi qu'une série d'entretiens avec des personnes souffrant de douleur chronique et ayant consulté dans l'un de ces centres. Les analyses de cette enquête ethnographique sont ancrées dans une réflexion socio-historique visant à reconstituer les transformations sociales et conceptuelles qui ont entraîné une nouvelle représentation de la douleur et " l'invention d'une médecine de la douleur ". Confrontés à cette innovation, les pouvoirs publics sont dans une position difficile : faut-il créer une nouvelle spécialité médicale (l'algologie), développer de grands centres de la douleur pluridisciplinaires dont l'activité de soins serait organisée en différents secteurs (douleurs aiguës, douleurs cancéreuses, douleurs du sida, douleurs chroniques) ? Les réponses à ces questions seront des réponses politiques, inscrites dans un débat plus large sur les transformations et l'avenir de la médecine. Elles nous concernent tous.

For any country that has a large and diverse migrant population, it is a struggle to connect these people to the country’s institutions, including the healthcare system, which can be overwhelming in its complexity. Cultural and language barriers often make it difficult for doctors to fully understand the symptoms of their migrant patients, reach accurate diagnoses, or properly treat their suffering.
