Excerpt from The Thyroid Gland in Health and Disease A period of sick leave from active service has afforded me an opportunity to incorporate into one volume the results of some fourteen years of study of the thyroid apparatus and its aetiology. When, in 1902, I undertook the investigation of the aetiology of its maladies a maze of hypotheses and uncertainties surrounded the subject. Knowing nothing, premising nothing, I determined to examine it from the combined epidemiological, clinical, experimental and pathological points of view. Some years of extensive epidemiological research cleared the ground of many false hypotheses, resolved some uncertainties, and brought to light certain facts which suggested an infectious or toxic origin for some of the thyroid's most common disorders. Clinical experience during the same period added further facts which strengthened this supposition; observations on man and experimental work on a large scale on animals converted the supposition into what I believe to be fact, and pathological studies have since confirmed my belief. Meanwhile it became necessary to revert to the study of the normal thyroid gland in order fully to understand the various processes of abnormality. As a result of this study a number of important points relating to the glands normal range of physiological action were elucidated, whereby it became possible to interpret the pathological changes to which various experimental procedures had given rise. The epidemiological work received confirmation from the observations made on artificially-bred trout in America, by Marine and Lenhart and by Gaylord; the clinical and therapeutic facts were supported by the researches of Messerli; the earlier experimental findings were confirmed by Sazaki, working under Wilms of Heidelberg, by Gaylord in America, and by Messerli in Switzerland. In 1956, three groups independently reported evidence that some thyroid disease appearing spontaneously in humans or experimentally induced in animals are related to autoimmune processes. The interval between these landmark discoveries and the present has witnessed a remarkable and continuing growth of both knowledge and concepts concerning the mechanisms of immune regulation, the pathogenesis of autoimmune thyroid diseases, and their clinical and laboratory manifestations. More importantly knowledge of thyroid autoimmunity has, in many respects, comprised the vanguard of an ever increasing appreciation and understanding of autoimmune diseases in general. On November 24-26 1986, an International Symposium on Thyroid Autoimmunity was held in Pisa. Its purpose was to commemorate the birth of thyroid autoimmunity as a scientific discipline, to summarize current knowledge and concepts in this area, and where possible, to anticipate areas of opportunity for the future - hence the theme of the Symposium, Memories and Perspectives. To open the meeting, the Magnifico Rettore (Chancellor) of the University of Pisa granted special Awards to Dr. Deborah Doniach, Dr. Ivan Roitt, and Dr. Noel R. Rose, who published the first fundamental studies in the field of thyroid autoimmunity, and to Dr. Duncan G. Adams, whose discovery of the long-acting thyroid stimulator (LATS) opened the door to our current understanding of the pathogenesis of Graves' disease. A discussion in extenso of the clinical syndrome must be left to the neurologist; suffice it to say that more intimate knowledge of the function of the several constituent parts of the gland must be acquired before a clear and concise clinical picture can be drawn representing various expressions of disease arising from lesions of one part of the gland or another. Of the two constituent parts, the anterior lobe is probably the most important; its secretion empties directly into the circulation, while the secretion from the posterior lobe empties into the cerebrospinal space. The latter secre tion contains certain active principles, though of much less importance to the maintenance Of the physiological equilibrium than the secretion of the anterior lobe. In general, it may be said that the various clinical manifestations are the result of hyperactivity on the one hand, and hypoactivity on the other, perversions of physiological activity similar to those Observed in diseases of the thyroid gland. Hypoactivity of the hypophysis leads, as does hypoactivity of the thyroid gland, to various disturbances of development and growth. Picture the myxedematous child on the one hand, and the short, excessively fat, growing child or young adult with the stigma of undeveloped geni talia and other defects. In marked distinction to this expression of hypophyseal disease, we have the antipathy of hypoactivitv in the hyperactivity of acromegaly.
