Introduction : la surveillance glycémique a connu une révolution ces dernières années avec l'arrivée du système CGM (Continuous Glucose Monitoring) basé sur la mesure en continu du glucose dans le milieu interstitiel. En 2019, 2 dispositifs sont remboursés en France chez le patient DT1 ; le Freestyle Libre (FSL) et le Dexcom G4 (DG4). Le DG4 se différencie du FSL par la présence d'alarmes sonores prévenant le patient d'une hypo- ou hyperglycémie, et semblerait donc plus adapté aux patients présentant une problématique hypoglycémique et/ou une variabilité glycémique importante. Si au travers des grandes études randomisées ces 2 dispositifs ont prouvé leur efficacité sur le contrôle glycémique et la qualité de vie des patients DT1, à ce jour les données comparatives ou de switch sont limitées. Nous avons souhaité étudier l'évolution de l'équilibre glycémique et de la qualité de vie après un changement de capteur (du FSL vers le DG4) sur une période de 3 mois. Matériel et Méthodes : nous avons réalisé une étude rétrospective monocentrique portant sur 10 patients DT1 inclus dans programme d'ETP réalisé dans le service du Pr D.RACCAH. Les données de l'AGP et les réponses aux questionnaires standardisés évaluant la qualité de vie (ADDQoL), la satisfaction vis-à-vis du dispositif (DTSQs) et le ressenti et la peur des hypoglycémies (score de Gold, score de Clarke, HFS) ont été recueillies à M0 et M3. Résultats : au total, les 10 patients inclus dans notre étude présentent une durée d'évolution de DT1 de 25.5 ans, une Hba1c dosée à 8.47%, traités dans 90% des cas par pompe externe à insuline. 3 patients ont connu un épisode d'hypoglycémie sévère sur l'année écoulée, et 50% des patients présentent un trouble de ressenti des hypoglycémies. Premièrement, il est retrouvé une augmentation significative du TIR (+11.77%, p=0.01), une baisse du temps passé en hypoglycémie
