Contexte : En 2012, la municipalité de Hem (Nord, 59) a développé un projet gérontologique ; un des axes de travail était le soutien du maintien à domicile. L’objectif de cette étude était d’établir une cartographie de la dépendance physique des personnes de 75 ans et plus vivant à domicile à Hem afin de situer les besoins selon les quartiers. L’existence d’aides pour les activités de la vie quotidienne a été évaluée et comparée à l’offre de soins. Méthode : Une étude descriptive par questionnaires a été menée de mars à juin 2013. Chaque Hémois de 75 ans ou plus a reçu un courrier de la municipalité l’informant de l’enquête. Un questionnaire contenant l’échelle d’évaluation de la dépendance ADL (Activities of Daily living) était joint. Les patients se rapprochaient de leur médecin traitant, informé par courrier, lequel était chargé de remplir le questionnaire. Résultats : 418 questionnaires ont été retournés sur 1309 déployés. Le taux de dépendance était à 2,93%, comparable dans les 8 regroupements de quartiers. Les habitants des quartiers 3 Fermes/Hauts Champs et 3 Baudets/Beaumont avaient plus de difficultés à se déplacer. 66,10% de la population bénéficiait d’aides pour les activités de la vie quotidienne. Celles-ci étaient plus fréquentes dans les quartiers 3 Fermes/Hauts Champs, 3 Baudets/Beaumont et Vallée 2. Conclusion : Il n’a pas été démontré de différence significative du taux de dépendance physique globale selon les quartiers. La répartition des professionnels de santé n’était pas homogène ; l’uniformisation de l’offre de soin à Hem est souhaitable.
