Introduction : La nouvelle loi Clayes-Leonetti de 2016 introduisant la possibilité de sédations en fin de vie répond à une demande de la société d'améliorer ces derniers instants et notamment de pouvoir mourir au domicile. La méconnaissance de cette loi couplée à l'évolution de la pédagogie en médecine a incité la Direction Générale de la santé à proposer à la SFAP de réaliser une formation en ligne gratuite, concernant les sédations, à destination première des médecins généralistes. Objectif : L'objectif de ce travail de thèse a été de réaliser une revue de la littérature pour mettre en place un outil de formation en ligne intitulé « Initiation aux pratiques sédatives », adapté à la pratique de la médecine générale en libéral. Méthode : Nous avons effectué une revue narrative de la littérature entre 2011 et 2020 sur l'évolution récente de la pédagogie en médecine, l'intérêt du e-learning dans l'apprentissage en médecine et les besoins en formation en soins palliatifs des médecins généralistes. Résultats : Les médecins généralistes sont demandeurs de formations en soins palliatifs et le e-learning apparait comme un bon outil pour diffuser des connaissances au plus grand nombre sans contraintes de temps ni de lieu. Sur les 1598 professionnels de santé ayant participé au e-learning « Initiation aux pratiques sédatives » depuis novembre 2019 (date de mise en ligne), sur une durée de 20 mois, 20% d'entre eux (328 personnes) ont terminé les 3 modules et ont évalué la formation. Les 98% de ceux qui ont terminé la formation l'ont appréciée « en grande partie » ou « totalement ». La plateforme et les différentes fonctionnalités ont été comprises « en grande partie » ou « totalement » par 97% des professionnels. Parmi les professionnels ayant complété la formation 90% ont jugé qu'ils avaient acquis « en grande partie » ou « totalement » de nouvelles compétences avec un impact sur leur pratique quotidienne. Les objectifs pédagogiques ont été considérés atteints « en grande partie » ou « totalement » par 97% des professionnels. Au total, 317 personnes soit 97%, ont jugé que cette formation avait un intérêt pour leur pratique professionnelle. Conclusion : Si aucune étude n'a encore mis en évidence la supériorité du e-learning par rapport à l'apprentissage classique en présentiel, concernant son impact auprès des patients, l'évolution actuelle des pratiques tend à généraliser ce type de formation en ligne. Les professionnels s'approprient aisément ce mode d'apprentissage et le propage rapidement autour d'eux. Néanmoins certains aspects de la prise en charge de la fin de vie ne peuvent s'apprendre de manière dématérialisée. Un complément de formation en présentiel encadré par des professionnels de soins palliatifs serait nécessaire au développement des compétences relationnelles liées à de telles pratiques.

These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain.
