Une étude prospective a été menée au sein de la consultation traumatologique post urgences du SAU Saint Antoine, pendant un mois, et portant sur l'ensemble des consultants. Nous avons cherché à évaluer l'intérêt de cette consultation ainsi que l'impact, sur la qualité des soins, d'un service de traumatologie en appui d'un service d'urgences. Pour ce faire, nous avons comparé les prises en charge initiales et l'évolution de ces patients suivant leur parcours de soins traumatologique initial. Nous avons ainsi pu distinguer 3 populations : la première population constituée de patients adressés par un service d'urgences appuyé par un service de traumatologie, la seconde population de patients adressés par les autres services d'urgences, et enfin la troisième population par des patients adressés par leur médecin généraliste. Les résultats confirment l'influence déterminante, sur la qualité des soins prodigués, d'un service de traumatologie en soutien d'un service d'urgences. Ces patients ont également été interrogés sur le jugement qu'ils portent sur la prise en charge traumatologique en médecine de ville. Très sévères, ils entérinent la démission des généralistes et justifient leur recours systématique aux urgences. Au vu de ces résultats, il apparaît clairement que la formation des médecins, et notamment ceux de l'urgence en traumatologie se devrait d'être renforcée et entretenue. De plus, toute décision de regroupement de services hospitaliers devrait être avant tout médicalement pesée, pour répondre à la double exigence de soins de qualité, mais aussi d'égalité d'accès à ces soins de qualité.

Objectif : L'objectif de cette étude est de montrer l'intérêt des consultations post-urgences (CPU) réalisées à Saint-Denis de la Réunion. Matériel et méthode : Nous avons réalisé une étude épidémiologique descriptive prospective sur les consultations post-urgences (CPU) assurées par les chirurgiens attachés aux urgencesà Saint-Denis de la Réunion durant la période du 11/02/2014 au 01/04/2014. Tous les patients se présentant en consultation post-urgence étaient inclus, aucun critère d'exclusion n'a été appliqué. Le critère d'évaluation principal était de montrer l'intérêt des consultations post-urgences au travers des redressements diagnostics réalisés. Pour alimenter notre raisonnement nous avons dans un second temps réalisé une enquête téléphonique sur 59 centres hospitaliers, dans le but d'identifier leur gestion de ces CPU. Les objectifs secondaires étaient d'identifier les patients à adresser en CPU et les médecins devant assurer ces consultations. Résultats : 121 patients ont été inclus. Nous avons suivi au total 198 CPU. Les patients étaient âgés de 17 à 88 ans. La moitié des patients étaient adressés en consultation post-urgence pour une réévaluation radio-clinique, un quart pour une réévaluation clinique simple, 14% pour une surveillance de plaie et 10% pour une réévaluation après la réalisation d'examens complémentaires en externe. 52% de ces consultations s'attachaient au suivi des fractures non chirurgicales, 25% à celui des entorses. L'intérêt de ces CPU a été mis en évidence par les 9% de redressement diagnostic effectué en CPU par rapport au diagnostic posé au service d'accueil des urgences. Nous avons également pu identifier les pathologies à adresser en CPU telles les entorses, les fractures, les plaies complexes et les contusions pour lesquelles un doute diagnostic persistait à la sortie des urgences. Les médecins urgentistes ne semblent pas être les acteurs de! choix de ces consultations. Par contre, il leur incombe de les organiser afin d'assurer un suivi optimal des patients des urgences. Les pratiques sont très disparates d'un centre à l'autre, avec 20% des centres organisant des CPU assurées par les urgentistes (9% dans les centres universitaires, 30% dans les autres). Ailleurs, les patients sont orientés vers les médecins généralistes, les médecins du sport ou les orthopédistes. Conclusion : Cette étude montre un bénéfice des CPU dans le suivi, après leur passage aux urgences, de ces 121 patients victimes de traumatisme orthopédique que ce soit dans le suivi des fractures, des entorses et même des contusions où les diagnostics posés aux urgences sont parfois corrigés. Il semble désormais important d'organiser ces consultations en identifiant les praticiens les plus à même de les assurer et de rédiger des protocoles écrits pour les médecins traitants pour les orienter dans la gestion de ces patients et leur proposer des liens médicaux spécialisés qui viendront compléter leur réseau habituel.

Neglected finger injuries may lead to chronic lesions that often have detrimental consequences for the practice of a ball sport, whether at a recreational level or at the top athlete level. The initial injury management is crucial. Depending on the type and the severity of the lesions, different people may get involved: the players themselves or their entourage, physiotherapists, club doctors, emergency physicians, family doctors, sports doctors or hand surgeons. Treatment is usually conservative: the long fingers require rapid mobilization to prevent stiffness and contrary to that, the thumb requires stability. Surgery may be necessary to reach these goals especially for athletes, because of the demands of their sport. Chronic lesions are also in part related to repeated trauma, requiring specialized long-term multidisciplinary treatment. Whenever radioactivity is released to the atmosphere, for example by the detonation of nuclear weapons or the testing of nuclear weapons or from nuclear reactor accidents that fraction of it which remains airborne for more than a few hours is liable to be attached to aerosol particles.
