Objectif : l'objectif de cette étude était d'évaluer l'impact de la propre implication des survivants d'un cancer dans la préparation de leur retour au travail sur les conditions de leur reprise. Méthode : un auto-questionnaire a été rempli par 105 survivants de cancer au moment de leur visite médicale de reprise du travail. Différentes modalités de reprise du travail en fonction des caractéristiques professionnelles et médicales ont été comparées par des tests statistiques multivariés. Résultats : la reprise du travail moins de 1 an après le diagnostic est associé à une visite régulière du salarié dans l'entreprise au cours de l'arrêt de travail. L'absence de changement d'employeur ou d'activité professionnelle sont associés à la réalisation d'une visite de pré-reprise auprès du médecin du travail. Le changement des conditions de travail est associé au souhait du salarié d'une adaptation de son traitement à la vie professionnelle. Conclusion : l'implication des survivants de cancer dans la préparation de leur retour au travail apparait associée à l'adaptation des conditions de reprise.

Les employeurs ont besoin d'être accompagnés afin de réussir le retour au travail d'un salarié après un cancer ou une maladie chronique. L'objectif de cette thèse est d'élaborer et de valider un guide à l'intention des entreprises pour faciliter le retour au travail et le maintien en emploi d'un travailleur après un cancer ou une maladie chronique. Dans cette même optique, un aide-mémoire pour le médecin du travail a été réalisé, concernant le cancer du sein. Evaluation à partir d'un questionnaire en ligne avec des réponses catégorielles et ouvertes. Pour le guide pratique employeur, 22 personnes ont participé à l'étude, représentant différentes catégories professionnelles en lien avec le maintien en emploi (médecin du travail, assistante sociale, psychologue, acteurs de l'entreprise ...). Pour l'aide-mémoire pour les médecins du travail, les 18 participants à l'étude étaient uniquement des médecins du travail. L'évaluation de ces deux documents a permis d'effectuer les modifications nécessaires pour améliorer leur facilité de compréhension et d'utilisation, leur utilité perçue et leur pertinence. Le guide pratique employeur est un outil innovant à destination des acteurs de l'entreprise concernés par le retour au travail et le maintien en emploi d'un(e) salarié(e) affecté(e) par une maladie chronique ou un cancer. L'aide-mémoire pour les médecins du travail est un outil visant à structurer l'évaluation de ces médecins durant la visite de reprise ou de pré-reprise après un arrêt de travail pour cancer du sein. Ces deux outils s'inscrivent dans le cadre du développement de l'intervention FASTRACS (faciliter et soutenir le retour au travail après un cancer du sein), qui est en cours d'évaluation au moyen d'un essai randomisé contrôlé multicentrique.

Chaque jour en France, plus de quatre-cents personnes actives ou en âge de l'être apprennent qu'elles ont un cancer. Or, plusieurs études françaises et internationales mettent en lumière l'impact négatif de la maladie sur la vie professionnelle, ce qui positionne le sujet du retour au travail et du maintien en emploi après le diagnostic d'un cancer au cœur d'enjeux sociaux et politiques, puisqu'il constitue un des objectifs du 3ème Plan Cancer. Cette recherche a pour objectif de participer à la compréhension des difficultés rencontrées par ces personnes dans la poursuite de leur vie professionnelle en dépit des dispositifs disponibles. Afin d'apporter une vision complète du sujet, cette recherche s'inspire de théories de santé publique, sociologique mais également économique, et mobilise des méthodes mixtes alliant analyses quantitatives et qualitatives. Nos résultats corroborent ceux de la littérature : les personnes ayant les caractéristiques socioéconomiques les moins favorables sont les plus vulnérables face au maintien en emploi à distance du diagnostic. En complément, l'approche individuelle souligne le rôle actif du patient tout au long du processus de retour au travail. Néanmoins, la capacité individuelle à se maintenir en emploi est altérée par l'accès socialement différencié aux dispositifs d'aide. Ainsi, cette thèse plaide pour un accompagnement personnalisé des personnes atteintes de cancer qui doit être réalisé précocement afin de prendre en considération simultanément les inégalités socioprofessionnelles et les motivations individuelles.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
