Les compétences affectives persistantes, malgré la maladie, amènent à s'interroger sur la question générale des bases affectives de la vie psychique. A partir de là, les défaillances observées chez les malades d'Alzheimer permettent de faire progresser notre représentation du fonctionnement psychique général, tant normal que pathologique. L'observation clinique conduit, de plus, à poser la question de la communication implicite et celle de la fonction des symptômes. Cela concerne certes les malades, mais aussi par extension les relations de la vie quotidienne. Une seconde partie s'intéresse au problème du placement de ces personnes en institution. A partir de là, se pose la question du fonctionnement des institutions, la façon de l'améliorer et de concevoir des institutions spécifiques. Un éclairage est également apporté sur les petits groupes de sociothérapie contribuant à un soin relationnel renforcé. Ce travail sera apprécié de l'ensemble des intervenants en quête de repères pour exercer au quotidien dans le respect de la dignité de tous.
