Bronchoscopy and Esophagoscopy A Manual of Peroral Endoscopy and Laryngeal Surgery by Chevalier Jackson [17] CHAPTER I-INSTRUMENTARIUM Direct laryngoscopy, bronchoscopy, esophagoscopy and gastroscopy are procedures in which the lower air and food passages are inspected and treated by the aid of electrically lighted tubes which serve as specula to manipulate obstructing tissues out of the way and to bring others into the line of direct vision. Illumination is supplied by a small tungsten-filamented, electric, "cold" lamp situated at the distal extremity of the instrument in a special groove which protects it from any possible injury during the introduction of instruments through the tube. The bronchi and the esophagus will not allow dilatation beyond their normal caliber; therefore, it is necessary to have tubes of the sizes to fit these passages at various developmental ages. Rupture or even over-distention of a bronchus or of the thoracic esophagus is almost invariably fatal. The armamentarium of the endoscopist must be complete, for it is rarely possible to substitute, or to improvise makeshifts, while the bronchoscope is in situ. Furthermore, the instruments must be of the proper model and well made; otherwise difficulties and dangers will attend attempts to see them. Excerpt from Tracheo-Bronchoscopy, Esophagoscopy and Gastroscopy The time has come when not only the profession but also the public demands that every laryngologist shall be expert at the removal of foreign bodies from the trachea, bronchi, esophagus and stomach. The day has come when the treatment of diseased organs, especially chronically diseased organs, without looking at them is regarded as a groping in the dark that is permissible only in organs that cannot be safely examined. The esophagus has been for some years granted, though somewhat grudgingly, a place among the organs to be examined. The trachea and bronchi, owing to the initiative of Professor Gustav Killian, have been recently accorded a place also. Lastly it has been the authors privilege to demonstrate the ease with which the stomach may be examined by endoscopy. At the present time, the only available information in the English language on these subjects is the reports of cases scattered through the journals. These reports do not give working data by which the student may learn how to proceed. This book is intended to furnish this information, and is not in any sense exhaustive. It is preliminary to a complete work, which the author has in preparation. While the author realizes that there are men more capable of writing on the subjects, yet, as they have not done so, this little book is offered with a full realization of its shortcomings, but also with the assurance that every assertion therein, not attributed to someone else, is the result of practical experience.
