The first laparoscopic cholecystectomy, performed by Prof. Philippe Mouret in 1987 and described by himself in the first chapter of this book, was an event that revolutionized surgery in the past few decades. Although the majority of surgeons today are unfamiliar with the his- ry of early minimally invasive surgery developments, it is important to realize that the advent of laparoscopy led not only to new surgical te- niques, but also to a change in the doctrine of medical care, by streng- ening the concept of minimal invasiveness. This is particularly the case for biliary lithiasis, for which laparoscopy has provided major benefits in terms of both diagnosis and surgical tre- ment. However, our understanding of the etiology of this disease, as well as the availability of effective medical therapies, have remained subst- tially unchanged over time. The application of the innovative approach has not been, in routine practice, without problems: between 1990 and 2000, the rate of serious complications from laparoscopic cholecystectomy were four times higher than those observed with open surgery. Even initial attempts at lapa- scopic treatment of biliary common bile duct stones resulted in excessive complications and associated medico-legal problems. Nonetheless, today, most indications have been stated, techniques have been standardized, and a new aspect to the treatment of biliary lithiasis has appeared.

Patients undergoing bariatric surgery are at high risk for both the prevalence of preoperative cholelithiasis as well as their postoperative development. The question of whether cholecystectomy should be performed during bariatric surgery remains controversial. The aim of this work is to study the results of the supra-selective approach for gallbladder management, involving concomitant cholecystectomy only in patients with gallstones-related symptoms. This is a retrospective review of a prospective database of all patients who underwent laparoscopic or robotic bariatric surgery between 2010 and 2018 in a single institution. Five hundred twenty patients were included with a median follow-up of 37.5 months. The procedure was Gastric Bypass in 328 patients (63.1%) and sleeve gastrectomy in 192 (36.9%). Seventy-one patients had a history of cholecystectomy (13.6%). Sixty-four patients had preoperative lithiasis (14.2%). Four patients underwent concomitant cholecystectomy. Fourteen (2.7%) patients required an intervention for symptomatic lithiasis: 12/445 (2.7%) patients required subsequent cholecystectomies without complication. Two patients (0.4%) required access to the biliary tree for the treatment of choledocolithiasis after RYGBP and were managed through percutaneous transhepatic approach with a success rate of 100% and no complication.The rate of symptomatic lithiasis after bariatric surgery is low. The need for a subsequent cholecystectomy in the setting of the supra-selective approach is rare, 2.7%.

After the Second World War the number of biliary diseases has abruptly risen and lithiasis has become in many regions the most frequent surgical disease of the abdomen. At present, though there are perspectives of rational conservative treat ment and though endoscopic techniques permit removing some of its complications even without laparotomy, the basic treatment method still remains, and probably shall remain for long the classical surgical intervention. On the contrary, the indications for such intervention are always widening, as its risks become always smaller and the requirements for prevention higher. Many books have been written on the surgery of biliary ducts, but recent knowledge is growing so rapidly that it forces within short periods its new integration and critical assessment. Attention is mostly paid to interventions on the biliary ducts with their wide scope of problems and to the prevention and treatment of postoperative disturbances. New and more perfect diagnostic possibilities and some more suitable technical procedures as well require comparison and verification, introduce new aspects on old problems, and even change some established rules.
